Last year dude threw stuff on the tracks at 8th-14st derailing a downtown A train, years back downtown D train derailed at 81st Museum while express downtown. They don’t happen often, as native New Yorkers remember that downtown 4 train at Union square in 1991.
There was a subway train crash in 1991 on the Lexington ave line far worse that this. A train going way to fast derailed just before 14th street, crashed into the tunnel sides and pillars, killing several people and seriously injuring dozens.

Maybe the trains sideswiped each other after one of them partially derailed. I do not think they were on the same track. If they were on the same track then the work train would be operating under home signal block and the operator failed to be governed by the rules.
