My respondets will be the company employees who have direct connection with my research title.i.e production management. So what kind of survey question shall i prepared for my respondent.?

Hi Isaac, identifying factors for your research is a very important stage. I suggest reading the literature on the topic. I suggest not asking others for it, but rather diving in the literature (e.g. read on risk perception, fear, emotional responses). There are no shortcuts to success! You can do this, my friend!
