The nail bed propably got damaged times ago making the nail plate not attaching to the toe anymore. In this case from shortly after the nail root. Meaning that these nails will always grow funky like this. Hence, these nails will always require different care, and with this fine and tough gentleman especially, pro care is required as he's on blood thinners. One wrong nip with the side cutter would not only create quite a mess but could actually put his live on jeopardy as him bleeding won't stop.
