From my experience counseling patients on these blood thinners I can tell you that healthcare professionals tend to make you aware of the worst (and common) side effects to keep the patients safe. I have high confidence that you will be able to fully recover to your baseline. Hopefully it will be nothing more than an extra task you have to complete everyday! You've made me a much better runner and whenever I need running advice this is my go to place. I'm sure many can say the same! I'm glad to see you're improving. Sage, I was 33 yo. when I ended up in the same room with patients who were old and had serious heart problems. It's not about odds. Things just happen. The secret is being merciful to yourself. It didn't happen because you were lucky before and others suffered. It happened because you had bad luck. One is not healthy by trying to be living healthier than anyone else 24/7. One is healthy because one lives a balanced life and still, sometimes bad things can happen and what doesn't kill you, makes you stronger. You will come back strong!

Sage, I was so sad when I watched your May 6th 2021 video when you first spoke of your symptoms. You appeared to have labored breathing even then as you tried to get through your video :( Although rare, without mentioning the "elephant in the room", it appears your first instinct of what may have triggered your PE may have actually been the elephant. Praying for you, Sage! I'm happy to see your spirit and body slowly improving!!
Years ago in medical school I remember being told that 80% of the conditions people suffer and die from are lifestyle related. After 25 yrs of practicing emergency medicine I can conclusively say that's accurate. Unfortunately, there's still 20% that's out of our control. But by living a healthy lifestyle you maximize the chances that you can overcome those unpredictable medical conditions that arise. In that sense we all live under the sword of Damocles, which is why everyone should practice gratitude and embrace every day. I agree with your cautious approach due to the blood thinner because I have seen all the serious bleeding events. Anticoagulants are arguably the most dangerous class of medicine prescribed. (The other contender would be opiates). Your risk of serious bleeding however is definitely lower than older patients who have weaker blood vessels and often multiple risks for GI bleeding. Lastly, I can't help but be curious about your hypercoagulable workup (Factor V Leiden, Protein C or S deficiency, etc). Two weeks out of the hospital I would expect that the results would be back. You've been very open about this event, but if you don't want to share such detail I understand. Hang in there, you will overcome!
Thanks for the info. Yeah, I guess I'm pretty worried about being in a car accident/getting hit hard...or just sitting here and having a brain bleed. I don't think I would be able to handle that very well and would probably die. So unfortunately they didn't do a hypercoagulable blood panel before putting me on Lovenox (first 5 days out of the ER/hospital) and now Pradaxa 150mg x twice daily. In Sept. I can go off the Pradaxa (at least for the blood test) so they can see if there are any genetic factors. Apparently that blood thinner messes with the results otherwise? Maybe I should have pushed for Warfarin...it was half the price!

Hey Sage - Good to see you back out on the trails. PE's are not completely uncommon for endurance athletes - don't beat yourself up. Amy Sproston - total bad ass ultra runner - had a PE a few years ago - perhaps you could reach out to her. If I remember, you also flew on a plane right before your PE - that is what happened to Amy. Living at altitude also presents more risk. Hang in there - you'll be back. Amy was back in top form months later.
@@GodzillaGoesGaga I would not say it is a cause - but endurance activities followed by long periods of sitting can increase the risk of DVT - which can lead to PE. I am not a medical professional and not entirely sure whether it is the sitting or the plane pressure that contributes to the risk. Compression socks or getting up frequently when possible helps.
Hey Sage you definitely make a good point at injury risk and blood thinners. Be careful out there! Don't be afraid to adjust your type of races if needed because I feel there's been several times in the past where you've taken some serious falls. And dude the pulmonary embolism has nothing to do with your diet, exercise routine, etc. It's either genetic, related to many of the long drives you do, etc. Hard to control ultimately.
Just now discovering your channel here after coming out of a submassive saddle pe at age 38. Not nearly as active as you i think, but i still lead an active healthy lifestyle before all this happened. You're doing great! I have had 5 c sections. I was a distance runner before, during, and now. But it takes so much time to heal. You've got this! You're body will heal!

Don't spread your anti vegan nonsense on this channel. The opposite is true, blood clots and embolisms of any kind are more common for meat eaters, as are all diseasese in general. This was a side effect of the vaccine. And his girlfriend also a vegan has had zero problems, as the millions of vegans out there who also have zero blood clots.
I'd say only in people low in vitamin B12 (which I am/was not)...that could increase a risk of a PE. So being low in vitamin B12 (which some vegans are) can be a risk factor. But generally people with all sorts of diets get pulmonary embolisms. If anything perhaps me being vegan saved my heart from being under more strain (or getting a heart attack). So I'd say being vegan plant based probably increases your chances of surviving a pulmonary embolism and having a faster recovery from one. Then again, I don't think diet has much to do with this at all (in terms of being the root cause). The truth is I don't know and don't have the answers though. There is probably also a higher correlation of endurance athletes getting pulmonary embolisms more often as well in general...but as we're taught in 5th grade science class: "Correlation does not imply Causation"
@@Len1977gt actually while it's possible to jump to conclusions about people's intent and such when your on these medications they will specifically tell you to avoid spinach, kale, and avocado because of the effects they have. That said, in normal quantities it shouldn't be an issue outside of treatments, but if someone is slamming spinach smoothies then that might be different. None of which means a vaccine is a problem, though, that's just people scaremongering. I'm assuming sage will be taking an INR reading regually now until off the thinners, so I'd be surprised if it wasn't mentioned to him. 👍 Also possible the thinner isn't warfarin and it doesn't have such issues.

II just got out of hospital after 5 days, I had submissive PEs in both lungs with heart strain and dilation on my CT :( on thinners now but I'm shattered and still short of breath:( I'm 22 and normally run 4 miles every day so I'm crushed :( I hope I can get back to being me with no long term heart issues I'm so scared :(
Glad you're back in running 👍 and saw your Strava updates, I believe you're on the good path but 4miler well under 8:00/mile pace is still faster than Tegla Loroupe's easy/recovery runs. There's no rush we followers we will wait better times just priority is to get you 100% and plus no matter how long it takes.

Really sorry to see what’s happened Sage, wish you a speedy recovery. One topic you could talk about is now you’re doing way less training are you having to think more about your weight and watch what you eat?
I also had/have a PE. It started two days after my first COVID vaccine shot with leg blood clots and eventually went to my lungs. On blood thinner and finally getting back to running. Hope to fully recover within 2 months. Will never take another COVID vaccine.
Hi Sage, it is encouraging to see you in a better shape, and recovering slowly but surely. Your open and honest vlogging is truly inspiring, to me and clearly to many others. Wishing you a full recovery and perhaps going beyond because as the saying goes, what doesn't kill you, can only make stronger: those mental challenges you are facing now and how you deal with them, may end up not only helping you out in life in general, but also for future races and any challenges you set for yourself. Oh, and by the way, it seems to me the crackling sound may be produced by your walking pole scratching/touching the floor while you are holding it tightly in the same hand as your selfie stick, the vibrations from the pole could be transferred through your hand to the stick and the camera.

I just had a lung Embolie. My blood clot was in the back of my knees. I felt pain for a week and just as soon as the pain dissappeared, i started having breath inssuficiency, chest pains. I could not climb stairs, take more than 10, steps. I had to call the ambulance to pick me up. They aspired the blood clots from the veins connecting the heart with the lungs with a catheter they inserted from the leg up. I am still under blood thinners and using a compression "pantyhose". I wonder when will i ever run again
10:20 it's a bit of a contradiction really, our bodies are what we need to exist, but there are times were we feel that it is holding us back. No worries, it's okay to think of such things once in a while, and come to terms with it as sort of a consequence as living, breathing things.

@@alanshrimpton6787 hi Alan, yes I made it to Hawks bay marathon, I had the injury for about a month before the race, but I tried training right up to the day, hoping it would come right on the day, was going to just walk it because still had IT Band pain, walked the first 2km but then jogged the rest of the way did it in 04:02:00 not the best time, not as good as last year Rotorua Marathon did in 03:23:35 just happy to get out on the trails & enjoy slowly getting better 😀

I’m about a week ahead with my PE recovery. Done a few runs of between 5k to 8k. Running much much slower than normal, but finding my Heart Rate is nearly in zone 4 for most of the run. It’s frustrating, especially not knowing how long it will be like this. Hope your recovery speeds along nicely.
hey sorry to hear about the PE and hope you are feeling better soon. My docs told me not to push hard when running (and to do things very gradually). I guess sometimes the clots takes several weeks (or even months) to really dissolve so there could still be some extra pressure on the heart from them. It really depends on the individual case though (and I'm obviously not a medial expert I'm just going by what the few doctors I've seen have told me in regards to getting back into running). Best of luck with the recovery process!

I think try shift your thought process to " am I not lucky this happened to me as a fit runner who eats well and looks after themselves" if you were predetermined to get this illness (genetics etc) you now stand a much better chance of recovering to 100% because you have looked after your self so well . Chin up man , least your alive to tell the tale . Not sure if you watched the Danish footballer Eriksen collapse during a football match recently , very fit and heart attack on pitch . This scares the crap out of me man. I have a question--it seems thrombosis is not completely unrelated to running/endurance sports. Typically, doctors will be less likely to think this is a potential diagnosis in a healthy, highly trained endurance athlete...or a fit individual with fairly good cardiovascular development. However, I've noticed that you are one of several I've seen at the higher levels suffer from DVT and/or a pulmonary embolism (typically as a result of DVT). I'm wondering, now, if on the contrary...extended high mileage running is potentially a risk factor for the development of deep vein thrombosis. There are several peer-reviewed case studies, as well as some general review papers examining this very question. It seems one plausible explanation is the upregulation of thrombotic factors initiated by extended levels of aerobic or endurance activity--this alongside the micro traumas and sometimes macro traumas faced by higher level endurance athletes/runners could be a potential causative factor in DVT development. Take this with the fact that it's common to want to immediately sit or lay down for extended periods following very high-mileage runs or races...it's not unreasonable to posit that the thrombotic cascade initiated by running followed by subsequent periods of disuse in the form of bedrest or sitting, could play a role in the development of DVT and potentially pulmonary embolism. When I saw your videos detailing this issue in yourself, it immediately sparked questioning...as it seems like another correlation--another extremely fit, high-level, active runner, suffering something that is supposed to be seen primarily in older, unfit individuals who often smoke, drink excessively and already suffer other comorbidities. I have a tendency to worry about my own health constantly...and at 34 years old who is reasonably fit...fairly lower body fat %...running 40-50miles weekly...I have noticed some strange issues in my legs that I don't know to attribute solely to the demands of my running OR because I may have a DVT somehow. I have noticed strange occurances at times...like a circular bruise right at the center of my shin after a long run that went away within 1-2 days. In same leg I noticed some minor redness and bruising right at the inside ankle. This same leg has what appears to be a bakers cyst behind the knee, which is particularly noticeable when my leg is fully extended. I don't have any pain...I do experience a significant degree of tightness particularly in the morning. I don't experience pain other than the normal amount one may endure running 10-15 miles at a time. I realize the only way to diagnose is to go to a doctor, but I am curious if you experienced similar strange things prior to you have the pulmonary embolism? Were there any signs of DVT that in retrospect you would have caught sooner prior to progression into a PE?

How getting tests has to be weighed against who's doing what for their profit vs your need. Sure you might earn a bit more but at what cost? Do you have any info about flying on a plane and the causes of PE. Is it similar to Decompression sickness from scuba and then flying ?

@@Len1977gt actually while it's possible to jump to conclusions about people's intent and such when your on these medications they will specifically tell you to avoid spinach, kale, and avocado because of the effects they have. That said, in normal quantities it shouldn't be an issue outside of treatments, but if someone is slamming spinach smoothies then that might be different. None of which means a vaccine is a problem, though, that's just people scaremongering. I'm assuming sage will be taking an INR reading regually now until off the thinners, so I'd be surprised if it wasn't mentioned to him. 👍 Also possible the thinner isn't warfarin and it doesn't have such issues.

II just got out of hospital after 5 days, I had submissive PEs in both lungs with heart strain and dilation on my CT :( on thinners now but I'm shattered and still short of breath:( I'm 22 and normally run 4 miles every day so I'm crushed :( I hope I can get back to being me with no long term heart issues I'm so scared :(
Glad you're back in running 👍 and saw your Strava updates, I believe you're on the good path but 4miler well under 8:00/mile pace is still faster than Tegla Loroupe's easy/recovery runs. There's no rush we followers we will wait better times just priority is to get you 100% and plus no matter how long it takes.

Really sorry to see what’s happened Sage, wish you a speedy recovery. One topic you could talk about is now you’re doing way less training are you having to think more about your weight and watch what you eat?
I also had/have a PE. It started two days after my first COVID vaccine shot with leg blood clots and eventually went to my lungs. On blood thinner and finally getting back to running. Hope to fully recover within 2 months. Will never take another COVID vaccine.

Hey sage, thank you for sharing your story, I am in a similar situation I was diagnosed last week with a PE and I'd always use to be big into fitness and this is an ass kicker. How'd you get your embolism and how's your recovery been?

I just had a lung Embolie. My blood clot was in the back of my knees. I felt pain for a week and just as soon as the pain dissappeared, i started having breath inssuficiency, chest pains. I could not climb stairs, take more than 10, steps. I had to call the ambulance to pick me up. They aspired the blood clots from the veins connecting the heart with the lungs with a catheter they inserted from the leg up. I am still under blood thinners and using a compression "pantyhose".

@@alanshrimpton6787 hi Alan, yes I made it to Hawks bay marathon, I had the injury for about a month before the race, but I tried training right up to the day, hoping it would come right on the day, was going to just walk it because still had IT Band pain, walked the first 2km but then jogged the rest of the way did it in 04:02:00 not the best time, not as good as last year Rotorua Marathon did in 03:23:35 just happy to get out on the trails & enjoy slowly getting better 😀

I’m about a week ahead with my PE recovery. Done a few runs of between 5k to 8k. Running much much slower than normal, but finding my Heart Rate is nearly in zone 4 for most of the run. It’s frustrating, especially not knowing how long it will be like this. Hope your recovery speeds along nicely.
hey sorry to hear about the PE and hope you are feeling better soon. My docs told me not to push hard when running (and to do things very gradually). I guess sometimes the clots takes several weeks (or even months) to really dissolve so there could still be some extra pressure on the heart from them. It really depends on the individual case though (and I'm obviously not a medial expert I'm just going by what the few doctors I've seen have told me in regards to getting back into running). Best of luck with the recovery process!

I ended up getting the Vaccine and it ruined my endurance going on a week now

I think try shift your thought process to " am I not lucky this happened to me as a fit runner who eats well and looks after themselves" if you were predetermined to get this illness (genetics etc) you now stand a much better chance of recovering to 100% because you have looked after your self so well .

This scares the crap out of me man. I have a question--it seems thrombosis is not completely unrelated to running/endurance sports. Typically, doctors will be less likely to think this is a potential diagnosis in a healthy, highly trained endurance athlete...or a fit individual with fairly good cardiovascular development. However, I've noticed that you are one of several I've seen at the higher levels suffer from DVT and/or a pulmonary embolism (typically as a result of DVT). I'm wondering, now, if on the contrary...extended high mileage running is potentially a risk factor for the development of deep vein thrombosis. There are several peer-reviewed case studies, as well as some general review papers examining this very question. It seems one plausible explanation is the upregulation of thrombotic factors initiated by extended levels of aerobic or endurance activity--this alongside the micro traumas and sometimes macro traumas faced by higher level endurance athletes/runners could be a potential causative factor in DVT development. Take this with the fact that it's common to want to immediately sit or lay down for extended periods following very high-mileage runs or races...it's not unreasonable to posit that the thrombotic cascade initiated by running followed by subsequent periods of disuse in the form of bedrest or sitting, could play a role in the development of DVT and potentially pulmonary embolism.
When I saw your videos detailing this issue in yourself, it immediately sparked questioning...as it seems like another correlation--another extremely fit, high-level, active runner, suffering something that is supposed to be seen primarily in older, unfit individuals who often smoke, drink excessively and already suffer other comorbidities. I have a tendency to worry about my own health constantly...and at 34 years old who is reasonably fit...fairly lower body fat %...running 40-50miles weekly...I have noticed some strange issues in my legs that I don't know to attribute solely to the demands of my running OR because I may have a DVT somehow.
I have noticed strange occurances at times...like a circular bruise right at the center of my shin after a long run that went away within 1-2 days. In same leg I noticed some minor redness and bruising right at the inside ankle. This same leg has what appears to be a bakers cyst behind the knee, which is particularly noticeable when my leg is fully extended. I don't have any pain...I do experience a significant degree of tightness particularly in the morning. I don't experience pain other than the normal amount one may endure running 10-15 miles at a time. I realize the only way to diagnose is to go to a doctor, but I am curious if you experienced similar strange things prior to you have the pulmonary embolism? Were there any signs of DVT that in retrospect you would have caught sooner prior to progression into a PE?
You've got this but don't overdo things. Let your body heal and listen to the Doctors and Nurses advice. They know what they are talking about!

How getting tests has to be weighed against who's doing what for their profit vs your need. Sure you might earn a bit more but at what cost? If you don't get shot in a mass murder you'll get robbed by healthcare vultures. Freedom isn't free right.
@@ironmantooltime its about money...nothing more. We here in the middle of europe have free healthcare, which might sound positive...but there is 1but. We have to pay like 13percent of our salary as healthcare insurance, and we have to, cannot choose if we want or not.
