    Too crippled to get the research done (half of which we already gave them the information on other forms) and their forms filled out, denied for disability.

    Page 1 says tell us about your work history since __. What date goes there?

      Often that date is filled out by SSA. Typically, it's related to a person's Alleged Onset Date and if a claimant has been working since they alleged they became disabled. (That date should be filled out by SSA).

      @@evansdisability It was mind blowing going to the local office. You were right, they don't care. It was not filled out and they said to put the date of disability after much argument. At first they said I was to fill out the first two pages, even got Hung up on on the phone ny a man who said "I've already told you, you fill out the first 2 pages and I have other calls." click. This is going to be a battle just to have clarification :(

    Why would someone receive this form back dated 12 years, as I did.
    I have not been self employed since 2013 ? Very confused it is now 2023 ..

      What happened with your claim? The same thing happened to me. I'm currently not self-employed.

    If I’m doing construction and my boss 1099 me he really is my employer but technically I’ll be a subcontractor cause I’m 1099 can I work more than 45 hours a month because I’m confuse I won’t be a business owner managing anything just a construction laborer paid hourly set pay for set hours
