Dear Dr. Mahboobian, can you explain in a video how you make sure no *_rotational deformity_* occurs during the femur lengthening, please? Specifically, when you cut the femur in two pieces, and insert the nail, how do you make sure that the original rotational alignment of the femur is preserved?
@Sero Suit Blocking screws are for "after the fact", meaning that once the surgeon makes sure the original alignment has been preserved, he fixates the orientation with the help of those screws. My question is about before using those screw: how can the surgeon make sure that the original alignment has not been altered.
@Sero Suit That is not possible. Just imagine for a minute, if you insert the nail and screw on both ends, then you won't be able to cut the bone !! So, what they do is that they rim the bone, then screw two markers in the bone, then they cut the bone, and finally place the nail and screws on both ends of the nail. The tricky part is that at the time of separation, those markers may not be able to preserve the original rotational alignment of the bone.
