Yes shadow bans are done by an AI system if someone gets reported too many times, permanent bans happen if ricochet detect any cheats being used or if a cod dev looks at your shadow ban and permanent bans you due to cheating.
The cod dev's can't get any access to your pc files or console as that would be illegal, they only look at your recent game history and stats and who reported you i'm pretty sure.
