    There is a PVC couplet in v3 and v4

      I believe both of those beats are atrial, conducting with a RBBB. If you look at the rhythm strip, the appearance of those beats are identical to all of the other conducted beats, They are not ventricular. Thanks for commenting.

    your explanation about axis is not clear at all. please explain little bit more clearly. it is a hard subject.
