Something of note. When Vex died, and the Raven Queen showed up. Vax's fate string was tied to Vex, and was cut. In this scene we see its now tied to the Matron of Ravens (1:45). She tells him not to fear death, because well, she is death, and he's apart of her now, just as shes apart of him.
I think its deeper than just “tied” to the raven queen. That his purpose was bigger than “i have to do this because i made a deal”. She asks if he accepts not “this is what you’ll do”
There's a later interaction when Vax becomes a revenant when the Matron takes off her mask, revealing her human face and the lonely expression that permanently adorns it. An interaction that makes her more relatable to Vax who has been shunned his whole life with none but his twin to fend off solitude.
it's a bit more complicated than that, there are multiple gods of death in D&D, the raven queen is a more "abstract" death goddess, she represents destiny, memories, the exact moment of your passage from life to death, the moment when you see again all your life passing before you, she is not death in the sense of the grim reaper, she is death in the sense of the moment when your hearth stop to beat and you expire THAT is the raven queen also, she is obssessed with the memories of dying mortals and pass a great deal of her time looking at the memories of random dead peoples
I thought of it as most fear not the act of dying it's self but the fear of not knowing what comes next, Vax need not fear death because he will return to the raven Queen when he dies.
Vax just came out of the temple of death covered head to toe in blood and acting happier than he’s been all season. If I were Vex I’d have worried he finally snapped.

They don't actually, personally kill people unless they absolutely have to. They protect the line between Life and Death, shepherd the souls to their afterlife, or next life.
My guess is that it is the blood of the people whose souls they have harvested. It would make sense due to her being death, which is why she tells Vax to not fear it.
