Great list! I would love to see a list of the twisted Wayne family alternates. For example, Thomas Wayne Junior who grew up to become Owlman,a member of the Crime Syndicate. We can't forget the Flash point Joker, Martha Wayne herself who went insane watching her son Bruce die in her arms in the alley instead of her and Thomas. And of course everyone's living nightmare, The Batman Who Laughs. Those are just the ones I can recall in recent comic pop culture.
It doesn't surprise me that Joker is number 1 considering his' reputation, he started out as a Clown-themed Criminal that was Super Hard to Kill, but he has since become more than just a mere man, now he is an influence made of Humanity's Darkest Nature, for example (If you haven't watched the show Gotham be aware of Spoilers): In the Gotham series we see a version of the events that eventually lead to Bruce Wayne becoming Batman, in the show Jim Gordon arrests a Psychopathic Murderer Called: "Jerome Valeska" whom is heavily implied to be the future Joker, however at some point Jerome dies, before that his' father (a Psychic) told him that the world would see him in it's nightmares forever, when the Criminals of Gotham see the Broadcast of Jerome's evil Schemes on TV a lot of them start laughing like they're all under the influence of some maniacal decease and they start to follow Jerome's example, it was even stated by the writers of the show that anyone could be the Joker, Jerome didn't even end up as the Joker despite everything we were lead to believe.

What makes Joker terrifying is that he can instill more fear in a country by himself than anything Scarecrow could ever make, has more charisma and presence than most world leaders and uses it without trying half the time, and lastly and most frightening of all is that he is smarter than almost EVERYONE else in DC and if he wasn't solely obsessed with Batman and circus themed crime he could destroy everything in a matter of days if not hours. Case and point, whenever he laughs, you as the audience know you shouldn't too but do so anyway.
The Arkham games and Batman Begins really did Scarecrow a disservice portraying him as a weakling who hides while his toxin does all the work. Scarecrow is actually ripped (he has body image issues from being bullied for being skinny, so he overcompensates) and he's a self-taught martial artist (a mixture of capoeira, crane style, and drunken boxing). He's gone toe to toe with Batman and held his own. Sigh.
Technically, all symbiotes (venom, carnage, etc.) as they dont possess “powers” (most of) the stuff they do is natural to them and is nothing special to other symbiotes,

Well, when Kingpin was first introduced in Spider-Man he had legitimate Super Strength. But then got toned down to more human levels when brought into Daredevil.
