XIPWNZz11 The upper hand as in getting the upper hand on her addiction, getting ontop of it, getting control of or perhaps beating it but lyric wise "she's under the upper hand" means by her keeping using she's keeping herself right beneath having the upper hand. I think Ed started to say he believed if she wanted she could of got the upper hand on her addiction. But the key was, If she wanted. And wanting to is Half of an addicts battle. Hope that makes sense.
Ed means shes at that point where shes about to get her shit together, yet always goes back to drugs because, as he says, "goes mad for a couple grams"
