    S4 (4th heart sound) is pathological. It is heard during Atrial Contraction which occurs during the Last Rapid Ventricular Filling phase when the atria has to do extra work incase of Restrictive Cardial Myopathy.

    Cardiac cycle-
    Isovolumeteric contraction
    S1 .
    Period of ejection-
    Rapid
    Slow.
    Protodiastole- S2
    Isovolumeteric relaxation
    Period of rapid filling
    Period of slow filling.
    Wriggler diagram-
    Phonocardiogram
    A wave- atrial contract
    C wave- bulging of av valve
    V wave- blood from vein.
    ECG-
    P wave atrial systole
    QRS wave ventricular systole
    T end of systole.
    Area under PV Indicates work done by heart...

    S4 is always pathological and it is heard when atrial contraction contributes to more than 20% filling. Seen in conditions like restrictive cardiomyopathy and long standing hypertension.

    S4 is the sound produced during atrial systole(rest 20% ) and it is seen in abnormal condition that is RCM (Restricted Cardiac Myopathy)

    S4 is the sound heard when atria is contracting so hard(for that left 20%), that a sound is generated.
    It usually happens in the case when the contraction of atria is less functional and some exercise has been done by the same person.

    In gyton there ia a statement at 3times the normal heart rate systole is about 0.65 of the entire cardiac cycle.
    I don't understand this one ...if the heart rate is increasing why is the systole duration also increasing
