Has definitely happened before with thousands of games being played everyday but this might be the first recorded and uploaded instance of it.
I mean how many rounds end in a trade? I know draws don't happen anymore but in my thousands and thousands of rounds I can probably count on one or two hands the amount of times I've seen it draw or otherwise the amount of 1v1s ending in a trade. Then add in the fact both players in that situation have to have gotten 4ks. Its plausible that this is the first time.

If every player has experienced what you've talked about even one or two times the entire time they've played, it is very likely this has happened before. The game has been out for 8 years now and if it happened to a content creator of all things after all this time, some random people have definitely experienced it or have had it happened and not even realized it. Gotta think about the hundreds of thousands if not millions of rounds that have been played throughout sieges' life span.
