Excellent explanation of the various treatments and their efficacy.
I’m 75 and had to stop running at 50 due to severe osteoarthritis.
I now cycle and/or go to the gym about 5 days a week.
I’ve run the gamut of treatments over the years:
PRP: Once a month for 3 months. No apparent effect.
Nerve ablation: Once - No apparent effect.
Steroid injections: 3 shots per knee every 3 months for several years. This appeared to have some benefit but not a dramatic improvement.
Finally, last year, I had one knee replaced and I wish I had done it sooner. I was back to work (desk job) and climbing stairs in 4 weeks.
I hope to get the other knee replaced as soon as I can get scheduled.
I’m using Tylenol and topical NSAIDs in the meantime.
I was not offered the viscoelastic injections but I would have liked to try them.
I do appreciate the summary of recommendations on the various treatment options as I’ve tried to minimize the use of oral NSAIDs and also try to cut down on the use of Tylenol in favor of Turmeric and topical CBD.
Thanks again for the informative and concise summary of available options.

Dr Peng: Thank you. I have arthritic knees, and have RA. To alleviate pain, I take pain medications from time to time. I can do these exercises without pain and within my endurance limits. I believe it is improving my conditions. I will also follow your video on lower back pain exercises. Thanks again.

Hello Dr.Peng…glad to have found these exercises (really can’t believe I want to find exercises! 😮). I have bone on bone on my left knee and am seriously considering knee replacement. The pain around the knee cap, the sharp jabs down the leg, the stiffness that prevents me from doing the stairs are all the stories you have heard.
I think I can do all of the exercises besides the bent knee on the floor and the prone one facing down on floor. Unfortunately, I have lumbar stenosis and I think that one will not be on my list.
I did 3 cortisone shots on my left and 2 on my right last year. In addition, I had the gel injections in December. It made my left knee worse.
Will watch your PRP video. Thank you!

Hi there I watch your videos which have been tremendously helpful. I have had arthroscopy on my right knee due to a torn meniscus. This was performed on the 11th of February 2024 unfortunately I am still in a lot of pain. The doctor said that this is because I have severe osteoarthritis which was an underlined problem that's why it is taking so long to heal. I have been for physiotherapy which helps and also take pain killers when necessary. I also swim in the sea every morning for 55 minutes some people say I shouldn't but I feel that it helps your opinion on this please. I am going to start your exercise routine tomorrow hopefully this will help too. 😊

Thanks for the good content; with a few modifications most could do these. Most people with arthritis wouldn't be able to stretch the quads like that. They'd have to usea strap, or soften the standing leg to allow more room for the other leg (and keep hips forward). Most likely they'd have to place the lower leg on the seat of the chair and then stop forward slightly to stretch the quad. I'm surprised you didn't offer these versions. The advanced level, they'd need a cushion under the knee to protect it.

I'm 76 years old very active woman - better shape than 40 year old; I would like to ask you by having PRP done then it's going to be ok for a year, by delaying the knee replacement I'll be older and would be difficult to have a surgery? What do you suggest ? I'm even thinking of Stem cell and what is your thoughts? Appreciate letting me know what you think. Also do you know someone in Denver Co? Thank you,

While these seem really simple (easy), I have such stiffness in left knee (bone-on-bone) that I cannot do quad stretch without getting muscle spasm. Also cannot get it high enough to hold. Finally, lying on stomach (at this point) is almost impossible. Can you address how those with really stiff, sore hip and back muscles can adjust (eliminate?) these exercises. I still do what I can but have a loooo-ng way to go.

I do full squats, climb 2 stairs at a time, run downstairs, do quad curls with 60 lbs, goblet squats with 30 lbs, walking lunges 40 lbs, walk backwards upstairs, ski, jumping jacks, burpees, work on the acreage around my home and refuse to sit more than 1/2 hr. I am 72, take NO pain meds, injections or otherwise. The only activity I cannot do that I did in the past is distance running which is ok because I now focus on strength. I want to take it further with Smith Machine squats, have some concerns with injuries. Please advise Dr Peng, no one takes me seriously.

These are great exercisesxand very well explained. Im a ballroom dancing teacher in my late 70s . Ive had àrthritis in one of my knees for more than a decade. A seriously damaged anterior meniscus and also cartilage damage. Every specialist ive been to has emphasised the paramount importance of keeping leg muscles strong, and i do this regularly at home, with gym work and also with my dancing. Dancing well requires the use of most of the leg muscles and also builds good balance and fluid controlled movement. ( but you have to work at it! You cant buy it in a supermarket)
The only exercise ive been advised to avoid is the quad stretch with the ankle pulled close to the buttocks. For me thats a definite no-no, but that may be just specific to my detailed problem. Ive had cortisone unjection into the knee before dance competitions. They do exactly what it says on the tin. But of course, it wont repair anything and can encourage movemenrs which ultimately cause more damage.
As part of my management i use anti- inflammatory gel 2% voltarol works best for me . Turmeric is good for overall systemic inflammation. Dont forget take it with some fat and a pinch of ground black pepper.

Hi Dr Peng, I’m currently 12 weeks post TKR and doing well, unfortunately my other knee with a lesser degree of OA has started to flair up again with severe inner side pain with any movement, I’m keen to avoid another replacement 😳 are these exercises ok/beneficial for both knees? Many thanks, subscribed. 🥰🏴󠁧󠁢󠁳󠁣󠁴󠁿🇬🇧

I like your explanation and videos. Could you please add some exercises for knee arthritis that could be done in the swimming pool. I don’t know swimming but do some simple exercises in the pool. Your expertise will help me in doing the right exercises. Thank you in advance
