I will answer all your greatest question on the movie here: 1.) He smiles at the end because: (1) he is partially mad from the brainwashing; (2) The bosses didn't come for him, they came for Pembroke. They were going to continue on their way to the facility, and with any luck might never leave; (3) He's just an evil looking dude in general, probably him trying to look elated that he had escaped all that. I think it is a combination of the above. Most likely just he's partially crazy still and elated so he smiled while thinking "Screw you bosses!" 1.5.) He has all his teeth also in the scene where hannah hands him back his ballerina figurine. THUS we can conclude that he had maybe all his teeth removed and replaced with dentures, or maybe just a tooth replacement/prosthetic. They showed that dental office full of dentures and such. He was brainwashed in that scene too, so that is why he had all his teeth [it's not because he was possessed by the main bad guy, you have watched too many demon/exorcist movies!]. 2.) The main bad guy was burnt badly (he had bandaged on in the old picture of the castle being built and lost his face). He has this long life elixir that allows him to heal, but not completely. Which is why he is green like an eel with no nose and has to make realistic face masks. 3.) All the workers are in on it because they gain long life/immortality due to the elixirs. Even officials in town are bribed. They can get as much money as they like owning such an elixir. 4.) A lot of people just fail to understand the whole process of how they were gaining "immortality" through use of the eels. Here is how it works: The eels usually live 12 years, but in their water they lived to be 300+. They were studying how to make humans also live longer. At first they ingested the juices from the eels (just the urine which would explain the yellow tint to the liquid). This liquid proved to be toxic. It caused chronic dehydration. NOW WHY would this liquid cause dehydration?! Well I have a B.S. in Biology with a Minor in chemistry (so let me explain the science), if you keep drinking your urine over and over it becomes concentrated and will cause dehydration in the end (if you keep drinking it). It can cause symptoms similar to those brought on by total kidney failure. WHAT HAPPENS during kidney failure? Your teeth can become loose and fall out!! Thus the water is yellow with the eels due to it being highly concentrated with eel urine. SO the facility is forcing eels and eel urine into the humans! Who are hallucinating and going crazy due to the chemicals, and also losing their teeth etc. Now the main idea is that the human body absorbs the bad chemicals and only lets out the good chemicals that cause "immortality," or long life. Still very concentrated so they only drink a drop at a time through those little tincture like glasses or they too would loose their teeth (though they wouldn't hallucinate probably as that is caused by the bad chemicals). Simply put: Eel urine = too toxic. Place eels and eel urine in humans, humans absorb bad toxins and urinate the elixir of long life without toxins for others to drink. They drink it, and live a long life. ANY OTHER QUESTIONS?
Pretty much all on the money except for one thing: the Baron's face wasn't on account of the eel transfusions, but rather just that he was horribly burned in the fire 200 years ago. That's why he was all bandaged up in the 1912 picture

Gore Verbinski and Justin Haythe are actually fan of Lovecraft's universe (they told us in a interview) and I think this story is a lot more attached to this universe than you think. According to Lovecraft, the evil is invincible and each time a character fights the Evil, he becomes mad and gains access to the truth. It is exactly that in "The cure for wellness". Each time Lockhart hallucinates (has a flash of lucidity in fact), he understands a part of the mystery. Each time he hears the handle of the toilet rings, he accesses to the next step of madness (and comprehension) : The guardian who takes the corpse to the cave, the falling tooth, ... But do you already seen an happy end in a Lovecraft story ? No, unfortunately. At the end, when Lockhart run away with Hannah, he has a sarcastic smile : it is not only because he is mad, but also because the Evil that Hannah killed (the dark spirit in Volmer's head) jumped into Lockhart's head. The new corrupted Lockhart is running away with Hannah, and he will make a baby with her 1 hour later. The Evil won at the End, not Hannah and Lockhart !! If you are not sure of this interpretation, two novels of Lovecraft is very close to this movie : "The Thing on the doorstep" and "the case of Charles Dexter Ward". Perhaps there are more elements in "the magic mountains" but I don't read it. What do you think about my interpretation ?
Yvan Goyard I disagree with your opinion on the ending kinda. They distroyed the Evil from the father, he woke up close to the end and found out the truth & then saved hannah baker. So i wouldn't exactly say the evil won, there wasn't any influence on lockhart during the movie to make him evil at the end. Everyone in here completely misunderstood the ending. The truth is that he never snapped back to reality. Him saving the girl, killing the guy, and riding off on the bike was all a dream - his overly teeth revealing grin gives that away because he lost some teeth through out the film. In reality, he's stuck with Eels in his system in a coma like state with all the other old people in glass boxes - like you see in the scene where's he's writing the letter and it cuts to the girl seeing him in the glass box, floating like Mr Pembroke. The hospital staff lied about the broken leg to help convince him that he needed to stay. It was never hurt in the crash to begin with. In reality, he never did any of those things you saw in the ending. It was all in his head whilst floating in glass box in the basement, a delusion brought on by the disease from drinking the water that gave him the halucinations about the Eels that led up to the ending..... Did you guys think he got out? How would he have gotten the Eels out of his body? It didn't explain that nor show us - because he didn't get them out of his system. He died there.

So, it's an exercise in nihilism. You finally figure out that you've been pointlessly chasing something all of your life, and then you ditch it and start chasing something else that has been deemed more 'meaningful' or 'fulfilling'. But really, it is just as pointless an exercise as chasing anything else, because nothing has purpose, or meaning, and we're all just temporary beings that fill up an uncaring universe, yada yada. So Lockhart has been obsessed with success his whole life and has chased that, just like his father. But then, he gets sent to the wellness center, where the Baron has been chasing longevity and purity, at the expense of, well, everyone. Finally Lockhart realizes that chasing success is a pointless, insane endeavor, as crazy chasing purity and longevity, and rides off on a bike with Hannah, ready to pursue happiness and truly living life. BUT verbinski shows that this is just as futile as every other endeavor, because a) Lockhart is smiling like a maniac, showing his insanity, and b) he has a full set of teeth, symbolic of his acceptance of the cure that isn't really a cure, because our existence is pointless, etc. (his teeth symbolize that because every other patient there also has a full set of teeth, even though their real teeth have been pulled out in the lung... but they still stay afterwards long enough to get dentures). Basically, Verbinski is telling us that we'd all be better off killing ourselves.
This movie was so weird and it got me thinking about it even days after i've seen it. There were scens where i really wished that i looked away, for example that *spoilers* scene with the teeth, I didn't expect the camera to be on it for so long haha, also the scene with the car hitting the deer was a bit over the top for me. Like seeing the animal struggling so hard was a bit weird to watch. Other than that, I liked the fact that it was suh a long movie and that it was an original script instead of another adaptation. Glad someone is posting about this movie on RUclips.. probably haven't seen much of those yet since the movie still has to come out in some parts of the world.

You guys are over analyzing this. At the end, he smiles because his evil bosses are heading for the castle, directly into the trap, and will get what they deserve. The bosses are extremely greedy and only care about the CEO's signature for the merger as the fallout guy. When they leave, they are headed towards the castle, Lockheart goes the other way, smiling about the impending doom of his evil bosses, and because he finally beat the Baron's game, he outsmarted the Baron.
I have a question about the actual ending. ***SPOILERS*** Why did Mr. Lockhart refuse to go with his old office bosses in the car and instead, rides away in the bike with Hannah, all wile smiling like a diabolical maniac? Where is he going and what does he plan on doing with Hannah?
I'm sure he has no diabolical plans for Hannah (I hope), I'm just sure he's just thrilled to get out of the business life (leaving his co-workers behind) since it forced his father to kill himself and it got mr. pembrock killed at the wellness center. I guess that smile is just him thrilled at the chance to start a new life.

Also he straight up did lose his mind after the incredible shock of having an eel swim down into his body. He started telling hannah why would anyone want to leave. Actually the ending was illogical but movie goers nowadays simply question nothing. As if they would fly 4 people of the company to Switzerland in the middle of the night, see a burning building and their heavily injured employee and just think: "What have you done. Get in the car."😂🤣 Why did they even drive there? Did they expect the reception of the building to be open for visitors at night? My theory is that there was actually nothing wrong with the facility and he indeed got crazy. We saw some of his memories, we saw his dreams of eel- Stuff before he ever even saw any. Think like that: His father jumped from the bridge. Maybe he looked down and saw him eaten by what he assumed to be eels. Then the women in the sanatory told him some story's from the past and his mind went berserk with it. Else everything wouldn't make any sense at all: Why should take Hannah to town being barefoot and not even asking if she had any shoes if he was "normal" at this point in the story? Why shouldn't they lock the rooms or seal them with a code? Why did they leave him alone in his room over and over again without any supervision despite causing trouble for the institution again and again? If the eels eat people why should they bring them into the water tank for his first therapy session just to gaslight him after despite all the other patients apparently knowing what's up and not having any issues with it because it's supposed to be healing? And I have douzends of more examples I am too lazy to type now. I don't know how anyone paying attention to every detail could really take all the details seriously despite contradicting each other every other minute. Either he's crazy and that's what the movie aimed for, or it was suuuper lazy writing just putting in whatever they could think of for shock value.

Honestly i though about this as well, but it opened up so many more questions that i gave up no it. But im willing to share them here
it reminds me so much of what is done to people in mental wards, covering up everything that happens to them and then doing horrible things to them , i hope some time soon more light will be shed on this

So, it's an exercise in nihilism. You finally figure out that you've been pointlessly chasing something all of your life, and then you ditch it and start chasing something else that has been deemed more 'meaningful' or 'fulfilling'. But really, it is just as pointless an exercise as chasing anything else, because nothing has purpose, or meaning, and we're all just temporary beings that fill up an uncaring universe, yada yada. So Lockhart has been obsessed with success his whole life and has chased that, just like his father. But then, he gets sent to the wellness center, where the Baron has been chasing longevity and purity, at the expense of, well, everyone. Finally Lockhart realizes that chasing success is a pointless, insane endeavor, as crazy chasing purity and longevity, and rides off on a bike with Hannah, ready to pursue happiness and truly living life. BUT verbinski shows that this is just as futile as every other endeavor, because a) Lockhart is smiling like a maniac, showing his insanity, and b) he has a full set of teeth, symbolic of his acceptance of the cure that isn't really a cure, because our existence is pointless, etc. (his teeth symbolize that because every other patient there also has a full set of teeth, even though their real teeth have been pulled out in the lung... but they still stay afterwards long enough to get dentures). Basically, Verbinski is telling us that we'd all be better off killing ourselves.

This movie was so weird and it got me thinking about it even days after i've seen it. There were scens where i really wished that i looked away, for example that *spoilers* scene with the teeth, I didn't expect the camera to be on it for so long haha, also the scene with the car hitting the deer was a bit over the top for me. Like seeing the animal struggling so hard was a bit weird to watch. Other than that, I liked the fact that it was suh a long movie and that it was an original script instead of another adaptation. Glad someone is posting about this movie on RUclips.. probably haven't seen much of those yet since the movie still has to come out in some parts of the world.

You guys are over analyzing this. At the end, he smiles because his evil bosses are heading for the castle, directly into the trap, and will get what they deserve.
The bosses are extremely greedy and only care about the CEO's signature for the merger as the fallout guy.
When they leave, they are headed towards the castle, Lockheart goes the other way, smiling about the impending doom of his evil bosses, and because he finally beat the Baron's game, he outsmarted the Baron.
Cure for wellness is to watch this movie. I don't feel well after watching it...

I have a question about the actual ending. ***SPOILERS*** Why did Mr. Lockhart refuse to go with his old office bosses in the car and instead, rides away in the bike with Hannah, all wile smiling like a diabolical maniac? Where is he going and what does he plan on doing with Hannah?
I'm sure he has no diabolical plans for Hannah (I hope), I'm just sure he's just thrilled to get out of the business life (leaving his co-workers behind) since it forced his father to kill himself and it got mr. pembrock killed at the wellness center. I guess that smile is just him thrilled at the chance to start a new life.
Did you notice the teeth grew back in his mouth when he smiled? How did he got them back?

My guess is if Lockhart lost his tooth after very short time that all those old ppl who may have been there for years loat all of their long ago.

what about the actual ending though where he rides off smiling like the fucking green goblin all sinister like? Also he straight up did lose his mind after the incredible shock of having an eel swim down into his body. He started telling hannah why would anyone want to leave. He seemed like he was crazy now like everyone else but then all of a sudden snaps out of it rescues hannah and kills the doctor then as he escapes he gives that awful evil smile. What does anyone have to say about those two things?

Actually the ending was illogical but movie goers nowadays simply question nothing. As if they would fly 4 people of the company to Switzerland in the middle of the night, see a burning building and their heavily injured employee and just think: "What have you done. Get in the car."😂🤣 Why did they even drive there? Did they expect the reception of the building to be open for visitors at night?
My theory is that there was actually nothing wrong with the facility and he indeed got crazy. We saw some of his memories, we saw his dreams of eel- Stuff before he ever even saw any. Think like that: His father jumped from the bridge. Maybe he looked down and saw him eaten by what he assumed to be eels. Then the women in the sanatory told him some story's from the past and his mind went berserk with it.
Else everything wouldn't make any sense at all: Why should take Hannah to town being barefoot and not even asking if she had any shoes if he was "normal" at this point in the story? Why shouldn't they lock the rooms or seal them with a code? Why did they leave him alone in his room over and over again without any supervision despite causing trouble for the institution again and again? If the eels eat people why should they bring them into the water tank for his first therapy session just to gaslight him after despite all the other patients apparently knowing what's up and not having any issues with it because it's supposed to be healing? And I have douzends of more examples I am too lazy to type now.
I don't know how anyone paying attention to every detail could really take all the details seriously despite contradicting each other every other minute. Either he's crazy and that's what the movie aimed for, or it was suuuper lazy writing just putting in whatever they could think of for shock value.

He smiles at the end showing that he still had his front tooth…. He is actually Dad who has cut off Lockhart’s face and is now masquerading as him. His grin at the end is maniacal as his daughter doesn’t realise that it is him and he now has what he wanted all along.

The movie is a metaphor for an investment fund, the water is the money, the contaminated water is the bad investment there is literally force into their throats, the potion are the profit of the fund, the real cure is satisfaction, and all the money of the world is not enough, in the end the fund is burned to the ground, and now he can escape, he is not high on money anymore, and he found someone he can care about, in other words he found the cure.
remember that all clients are rich people who are normally clients of big funds.

does anyone know the actual ending? the best I have been able to come up with is he is insane and this entire movie is playing out in his head. there are way too many faults in the story to simply be production error. biggest one is his smile at the end, his teeth feel out. for that matter his boss, he has x-rays of his teeth and all of his front teeth are missing but it never shows this he is always seen with a full set of teeth. this movie is not what it seems on the surface.

Then how are the others (sheriff and Hannah) are ok? 4. So those 'patients' are working with doc? Why are they like, saying 'I'm not sick' like crazy persons? If they were high, how were they drugged? 5. The scene when Hannah saw Lockhart's body in water like his CEO, what the fuck is that scene. There's two Lockhart? 6. How the hell Hannah knows the melody that Lockhart's mother knows? 7. If eels are carnivorous, why didn't they chew Lockhart up when he was stuck in giant tank? 8. Exactly what activity makes them infected? The steam bath? Water drinking? Weird barrel? Getting stuck in the water tank? Pouring eels right into their stomach? 9. So if they get infected, they get high and when they get juiced out, they turn into mummies? It looks like some people like that puzzle granny and Lockhart can get back well easily, so they are aware of their death, but keep doing it? 10. When Lockhart is dressed up, his pants are in his leg plaster, so he must've took his plaster off before, but didn't recognize his legs are fine? 11. Why is Lockhart's teeth are back when he is smiling in the end and why is he smiling

1. Because he wasn't dead. Lockheart assumed he was dead. Later in the movie you can see Lockhart in the same kind of water treatment.
May be the other reason of his evil smile at last is that he realized at last after all we are slaves of everything or materialistic things. So rejected to their boses and moved in his own way with his evil smile
