There is no antagonist more evil then a character with no personality and characterization the most evil antagonist is barcov for enslavement and torture of innocent people and children which of makarov only killed to make a message
I mean, we actually can see more of his backstory in the opening cutscene to No Russian. During it, you can see a bunch of texts that scrolls a few times, that text contains info on why he does what he does and the many things he's done, some of which includes hijackings, trafficking women, and multiple bombings and assassinations. It's chilling stuff
