I fractured my right elbow and dislocated my left elbow. The ER doctor had this big male nurse put his massive forearm on the elbow to use as leverage and rotate my forearm to 90 degrees. They warn me this is going to hurt and they were not kidding. Everyone heard the pop but the doctor did not think it was fully in. They again bend my left arm and I heard two little pops. Now the doctor felt elbow was put back in place. Looks like she had it easier than mine. Was not given any pain medication.

They gave me a sling and sent me on my way, as I am already on multiple medications for my chronic pelvic pain and conditions. Although today I cannot move this arm and slept very little do to the pain.

Only once have I been given ketamine and I must say it was a terrifying experience. They actually had to give me something to lower my blood pressure. Pre-procedure it was around 110/60.

LOL I was 12 years old then and had just toured Alcatraz. I not only apologized repeatedly after it was done, but brought them all flowers 3 weeks later.
Never ceases to amaze me why Dr's, with access to every analgesia from Paracetamol to Fentanyl have to put patients through UNNECESSARY pain & stress when they are about to perform a painful procedure. Without knowing how much this poor lady weighed, 80 - 100mcg of Fentanyl with another 100mcg 'on the ready' would have sedated her & relaxed the muscles long enough to reduce that dislocation in half the time.
Isaac Vivienne i went through that same procedure but with 14mg morphine i was still screaming with pain. 3 tries while i was fully conscious to put it back and when they didn't manage they put me on general anesthetic. would have been much quicker if they did that in the first place
I know this is an old video and you have probably forgotten the specifics Dr. Mellick, But- Why was she given a light sedation with low dose Ketamine instead of Propofol? Was it because of the risk factor due to her age? or did she want a 'light' sedation. It seems as though if she were sedated completely from the beginning it would have been a lot easier. Just wondering, These videos rock! but bring back you trumpet skills!
Yes, I reviewed the video and we did the low dose ketamine on this patient. This definitely needed supplemental opiates for pain control. Ketamine can be opiate sparing, but looking back, I think we probably would have done better going full dose or using propofol with it.
That's why I like the Ketamine/Versed combo. I like to knock em the hell out with an IM macro-dose, then use the Midazolam to control the emergent anxiety. That's just me though. I like to use that combo for a variety of things.
Adults and children may have dreams, a floating feeling, flashbacks, alterations in mood or body image. It may be cultural. I've heard from docs using it in New Guinea said their patients came out of the sedation singing hymns.
I'm curious. What's the advantage of giving the Ketamine and the morphine together? Does the low dose of Ketamine help the morphine do its job, or is the Ketamine just there in the mix to sedate her?

This exact thing happened to me and I'm 12, I fell off the high bar at gymnastics and my elbow was dislocated at a 220 degree angle, I have the worst kind of dislocation plus I'm double jointed witch makes it 10 times worse. It's so bad that I might need surgery because my tendence were stretched really badly. Because of all this I have to quit gymnastics. I'm in a cast for 1 month then a brace for 3 months then physical therapy for a while

When the elbow (or shoulder, or knee, or ankle, or etc) is over there when it should be over here, lets try NOT to say, "Ma'am you're going to HAVE to calm down!" or "Relax!"" or terms like that. We can say "If you take some deep breaths try to tough it out for a second" or "if you can help me by holding still for a second," but if mine is out of place and I hurt like hell please don't tell me to relax, or calm down.
just ry a subanaesthetic dose of K and you'll get the Idea... I was administered a small dose IV and it felt really like they mixed a sedative with a psychedelic and it really felt I was out of my body and visiting the afterlife... and it felt so real and there was that feeling that a ultimate truth was going to be revealed to me.

Ketamine is good but not perfect for pain control. In this case, the ketamine was used to reduce her anxiety.
