  • Dr Stuart McGill is a professor emeritus at the University of Waterloo and a world expert in back pain.
In the new WFH world, many people are changing their routines, desk setups and posture. Combined with limited access to gyms and less chance to move and exercise, this is a perfect storm for creating back pain.
Expect to learn the number one cause of back pain Stu sees in his patients, why most physicians are wholly unprepared to deal with spinal injuries, why social media can stop you from being a master of your craft, whether our ancestors suffered with back pain and much more...

I discovered Dr. McGill’s book Back Mechanic a couple weeks ago after doing research for my crippling back pain (going to the chiropractor only made it worse 2x, went to my doctor whoordered an mri- which my health Insurance has denied until I wait a mandatory 6 weeks and try other therapies ). So I was thrilled to see him on one of my absolute favorite podcasts! 😁 I’ve read the book, and have been performing the big 3 and walking on a daily basis for 1 week and my pain has decreased by like 70%. It’s a really hard pill to swallow to realize that something I have loved so much (Crossfit) has also contributed to a lot of pain. I see now that Cf coupled with a desk job and bad movement patterns in my everyday life was a foregone conclusion to constant pain. But I am very hopeful that if I follow the direction given in the book that I can be pain free. Thanks, Doc!

I wish I came across this video sooner. I'm a combination of both types - Completely lacking proper core control and everything was locked up. My lower vertebrae have compression wear and my hips were so locked up, eventually all of my muscles just sort of gave up and it felt like gravity was turned up 100x on my spine. Then it happened again. Again. Trouble sitting upright, bending over, walking. Locking up while walking, on ladders, and unable to roll over or get out of bed for days at a time. It just kept happening. At 28.
He's right, it's difficult trying to find proper help. I found a few decent professionals and cobbled together my own plan. The bulk of my plan is stretching and core. I had to learn that the core consists of much more than just my abs. Because my hips were locked up for so long, the muscles surrounding them (ex. hip flexors) were almost non existent. Chiro and massage helped a lot, trying different professionals helped me understand who is knowledgeable and there to help, and who is just looking for a buck.
Start a small routine. Commit to it. Add to it. Learn everything you can about your body and work within your limits. Yeah it sucks having to change my routine, to wake up earlier to exercise, but it sure beats painful immobility.

Chris... this was more than excellent. Two people that really understand the troubles and psychology of spinal injury. I am 42 and suffering more and more often. It’s more frequent and yes, it can be because of more sitting but its a vicious cycle. Pain causes you to rest. Rest is laying or sittng. When exercise might cause more flare-ups... it faces you in the wrong direction. We, the people must turn (for the most part) to the medical industries. I just got turned down from my insurance for an mri... because I hadn’t done at least 6 weeks of either meds or injections. If you have pain that occurs at diff parts of the spine, how would a doc even know where to inject the steriod?

Or are we sentenced to move like a robot for the rest of our lives? Isnt there a missing link in back pain treatment? Why does it stop at "avoid flexion and brace"?

I stand for most of the day and had to correct my posture to eliminate the pain and soreness I was feeling. I have flat feet and correcting that with foot exercises SIGNIFICANTLY helped my posture. Don’t stand duck footed either. You feet is your foundation. If it’s not properly supporting your weight, then your posture breaks down through the kinetic chain. It’s bound to break down though when wearing a lead vest the whole day, but it will help. Make sure your neck is properly aligned and not drooping when looking down.

Sorry but you dont hydrate your spine by drinking wather. Thats not how the spise works. When you Wake up In the morning, thats the time the spine is the most hydrated, and is one of the reason it’s feels stiff.🙂 and after an hour or so, walking around, the spine starts to dehydrate to normal levels.

In his book, McGill says that sleeping more than 8 hours can give you more pain because the spinal discs get filled with water and if you go over the 8 hour period they can worsen existing pain (whether it's a disc bulge or herniation) because the bulge can grow due to the excess of water. I haven't read or heard him say anything about lying down for long hours though but the same principles apply.
