Enlargement of the medial temporal lobe, including the amygdala, is sometimes seen as the only abnormality on MRI in the work up of epilepsy. It can be difficult to differentiate post ictal changes, hamartomas, developmental disorders or neoplastic lesions. This is a short overview of existing literature that has helped me to interpret and correlate the imaging findings in individual patients.
