Right before the second half, that's his "Happy" state. The second half is what Unknown Suffering represents in the story: Mickey's insanity. When the screaming kicks in, that's Last Day / the ending cutscene. [EVERYTHING FROM HERE ON OUT IS SPOILERS FOR PART 2] Then, the actual song's outro is the ending of Last Day, Mickey dead in the middle of the street. A beautifully tragic story that can be told via one song.
