Continue Care Facility Overview

Continue Care is a medication management and care navigation program to keep patients healthy at home.
Continue Care provides a suite of services, including pharmacy, in-home nursing support, virtual nursing support, and coordination of personal care to manage the full scope of patients’ health. Whether patients are referred by a provider or after discharge from a hospital or skilled nursing facility, we are with our patients every step of the way to ensure they receive the right care at the right time - when they need it most.
