It’s a night and day difference between what you looked like in the past and what you look like now. Really wish I had your side profile but I’ve been a mouth breather my whole life. I’m thinking of getting the surgery done now at 22. I chose braces when I was young instead of jaw advancement surgery since I was scared of surgeries. I regret it. Ended up with 6 missing teeth at the end of my treatment and I still hate the way my side profile looks, always have.
I was told when I was a child, the doctor wanted to break my jaw. Instead, a cap was put on my tooth for a certain amount of time. How would I find out if I needed jaw advancement surgery?
I had MMA only, but it didn’t cure my UARS. Looking at getting expansion within the next year. Do you recommend MARPE with surgical cuts or EASE? I’d prefer to get MARPE since EASE is so expensive.
MARPE or MSE didn't help me too, because my posterior nasal congestion blocks my breathing. I am reverse turning my MSE back and hope I can find a surgeon that can do EASE that I can afford.
i mean if you have the money, EASE for sure. hard to go wrong with Dr. Li. there's also FME, which is a newly released bone borne expander that has been making a lot of waves, but it's still a bit too early to say conclusively what its capabilities are

Hi!!! I just had my first ever sleep study done and am new to this whole space. I was diagnosed with mild apnea with an arousal index of 30. The recommendation was to get a CPAP machine. I am not a mouth breather but was wondering if this is the best option? As I would have to use the CPAP forever and am worried about it causing further irritation. It seems that surgery could be a permanent solution. Just looking for honest thoughts here!
It might end up being your best solution, but you have to try CPAP (and/or BiPAP and/or APAP) first. And you should probably try a mandibular advancement device (AKA “oral appliance”) first, too….after the CPAP, that is, assuming CPAP doesn’t do the trick for you. Don’t get discouraged. Take it one step at a time. Know that you might be having more than just mild sleep apnea if your arousal index is 30; not all tests can detect all breathing problems. If you want to learn a lot, check out Evan’s older videos, and listen to the podcast Sleep Apnea Stories (hosted by Emma Cooksey). You have to take the information in Sleep Apnea Stories with a grain of salt (I heard that Vivos appliances don’t work very well, for example), but there is a lot of good stuff to learn in there. I’ve binged the first 55 episodes so far.
How can we get started on getting with Lancoms? I have read time and time again that you need a referral from your ortho or dentist to even get a consult with them, did you do that? Who did you go to first?

MMA surgery will be ineffective, and you will continue to have sleep apnea if you continue to mouth breathe instead of nasal breathing when you go to sleep. The benefits of MMA will reverse itself, and mouth breathing will cause your airways to become narrower again even after MMA. If you continue to mouth breathe during sleep after MMA, your AHI will not get down to normal numbers. Mouth tape when you go to sleep, and you will see the AHI numbers get down to normal readings. AHI analysis is good, but alone, it's not the gold standard. Sleep studies at a sleep clinic instead of home studies are better because it breaks down what sleep cycle stages of sleep you achieved and how long you were in these stages. If the highest stage of sleep was 2 and you never got into stage 3 rand 4, then a low AHI means nothing. I speak from personal experience.
@evanlavizadeh4032 Oy vey! You got to be kid'n! You tell people to do the research, but evidently, you haven't and can't seem to back it up with any facts, so I will. A good percentage of sufferers, including myself, developed severe sleep apnea, starting as a toddler as a result of being a mouth breather while awake and during sleep. Biomedical J 2023 Jun 46 (3): 100536 2022 May 10. I dont know the origins of your sleep apnea, genetic or otherwise, but I'm glad MMA helped you. I can cite peer reviewed studies that show mouth breathing in adults causes sleep apnea. In toddlers mouth breathing is even worse because, as they age, it causes underdevelopement of the jaw and reduction of airspace in the oral cavity. In my case, my airspace width in the pharnyx was the size of a baby, 6 mm. The average adult is 12 to 14 mm. It also causes facial abnormalities such as long, narrow face, lack of cheek bones, crooked nose, and crooked teeth. Allergic looking eyes. This happens before your bones harden and grow to a adult. Obviously, sleep apnea increases AHI ( average number of apnea and hypopneas per hour), but it also disrupts sleep cycles mainly in stages 1, 2, and Rem resulting you from getting to stage 3 and 4. That's where deep sleep occurs, which is critical for your brain and body to recover and grow: Front Public Health 2022 10:929165. 2022 Sep. 8. I had UPPP surgery in 2008 and MMA surgery in 2010. After MMA surgery, my sleep apnea was far from being under control. It went from a 77 AHI to 26. After both surgeries, I never got higher than stage 2 sleep. I went back on cpap but that didn't improve my numbers. I started to tape my mouth up before sleeping and the improvement in the quality of my sleep was miraculous. Went for another sleep study this time with no cpap and only taping my mouth and my AHI score was under 5. My sleep doctor couldn't believe it but all the research made sense of it. My UPPP and MMA surgery was done by Dr. Nelson Powell and Dr. Robert Riley of Stanford University. Those 2 surgeons along with Dr. Kasey Li who use to work in the same office are world reknown and have written many peer reviewed research on MMA surgery. They had a 95% success record in this field. Dr. Kasey Li has said recently that after MMA surgery if you continue to mouth breathe during sleep and don't switch to nasal breathing then the MMA expansion will be reversed and the improvement from MMA will be diminished.
@@jefreywith1f924 much respect for the explanation! And citations! I appreciate that. Yeah I think I was responding to how dramatic your statement was but hey, I'm sold. Thanks.
This is likely my case as well. Low AHI but 0% REM and Deep sleep. Almost all of my in lab sleep study was light sleep. I guess that explains the severe lack of sleep I suffer from. But the low AHI complicates things. I know I am suffering from sleep apnea or UARS, and I consulted Dr. Kasey Li who agreed with me after looking at my scan and asking my symptom's, but without a positive sleep study it can be hard to convince doctors. I can't breathe through my nose during sleep almost at all. I am a candidate for expansion. Do you have any tips for me? Do you think I should try to get a diagnosis or just get the treatment that is indicated for me (expansion)?
@UsamaKhan-xs9do You say you mouth breathe during sleep. Do you also do it while awake thruout the day? And also do you mouth breathe out of habit or due to nasal blockage from a medical ailment? This may sound counterintuitive, but it is true that mouth breathing alone can cause nasal blockage. When you breathe thru your mouth, the brain thinks you're releasing carbon dioxide too fast from your nose, which triggers the production of mucus to slow breathing. The mucus blocks the nasal passageway and creates a bad cycle to breathe even more thru your mouth. Before I recommend mouth taping, you need to see an ENT to make sure nothing is preventing you from breathing thru your nose. Also, it's a good idea to get 3d imaging done of your pharnyx( throat)to see if it's of normal size. Also, it needs to be established whether your tongue is falling back during sleep and blocking your throat from getting air. Their are surgical procedures to prevent this from happening. Please address your nasal blockage issues first if any exist before you try mouth taping. A lot of so-called health experts say mouth taping is risky and dangerous, but I beg to differ, plus its a big financial loss for them. It gave me back my life when CPAP and surgery were ineffective. Just look at all the testimonies of sleep sufferers who buy specialized tape they purchase online for this very purpose. This tape was even pitched on the TV show Shark Tank, and it convinced Mark Cuban to be an invester in the product. The presenter produced studies to prove its effectiveness. For me, paper tape you buy in the store works just as well. All you need is one strip of tape over your lips diagonally or vertically, and even so it doesn't sound like much, that's all it takes. Just one single strip like a bandage will do the trick and will not come undone during the night. This is a practice conceived by Dr. Buteyko, who developed the Buteyko Breathing Method. He has healthcare professionals all over the world who teach people effective breathing methods. It's even used by some myrofunctional therapists.
@@evanlavizadeh4032 may be! who knows. But jaw now looks perfect. Hair procedure is much more easy to do. I actually started with hair. Now i need to fix the jaw and a bit affraid of it. @@evanlavizadeh4032 and that's funny! i'm having that nose issue. It's a bit difficult to breath. And doctors they said - "are you a boxer? your nose has deformation! Let's drill it or birn it!" But in fact it was a jaw! and nobody told me that! I'm almost 40 years old!
I can't say, because it's a case by case basis. Also, it depends on how your surgeon performs your MMA. Rotation, downgrafting, impaction, nasal floor recontouring. All these MMA options change your nasal breathing for better or worse. If you have a narrow palate and nasal cavity, then MMA alone will provide a suboptimal result. Personally, I feel very lucky that I was able to do both. That's as much as I can say, unfortunately.
@@evanlavizadeh4032 thx for the reply. Could you describe the diffrence in nasal benefits between mse and jaw surgery? In your previous video you mentioned that they both helped, but could you go into more detail on the diffrence between the 2?
@@kichki777 palatal expansion makes the nasal cavity wider and MMA makes the nasal cavity either more deep or more shallow. So they both change the size of the nasal cavity but along different axes.
@@kichki777 UJS surgery is part of MMA, so it all applies. Impaction shallows the nasal cavity which reduces nasal breathing benefits. And sure, that makes sense to me, but I did widening AND deepening. I don't think only one would have been satisfactory. If you have a narrow palate, you're leaving part of the equation out. And for some patients of MMA, you wont have a deepening of the nasal cavity, but a shallowing, which would make nasal breathing worse. You have to look at your virtual surgical plan. Then, you can estimate how much more or less volume your nasal cavity will have. It's a case by case basis.

I am concerned about not getting the lateral movement because of the Le Fort I cut with the MSE, but at the same time, I will be able to correct my teeth much sooner and have a normal bite and will be able to do mewing. Alternatively, should I go for DOME or surgical-assisted MSE followed by MMA, which will take much longer but might offer benefits from the expansion without interference from the Le Fort I cut. It seems to me that your breathing got much better after MMA rather than MSE
That's amazing. Has the amount of hours of sleep you require reduced too?

Maybe. I'm so used to sleeping on my side my whole life that it still feels weird to even try to sleep on my back.
I'm trying to find one true statement in your comment, but I can't find one.
@evanlavizadeh4032
Oy vey!
@UsamaKhan-xs9do
You say you mouth breathe during sleep.

I do not have the option of mse. Do you think mma alone can fix my nasal breathing ? Also what where your movement in the upper.
@evanlavizadeh4032 what would ujs and impaction do ? Wouldn't widening and deepening result in the same effect ? More air inhaled.
