What you said about "performing a mundane task to avoid death" in the beginning sparked an idea. Make a torture device that tries to kill the "patient" every minute or so unless they perform a simple and boring task (Or hard and complicated, just to tire them out faster) to avoid death. The task would have to be complicated enough though so that it can't be automatically completed with an auto clicker or macro.
The "patient" would eventually go insane from performing the same task over, and over, and OVER again to avoid death untill they give up and let themselves die. Imagine this in hardcore mode O_O
Wood planks slowly drop from the ceiling. When you get five, craft a boat and throw it in the hopper. If you don't, boom.

Perfect torture: make a redstone clock, add pistons, cacti, make it constantly go back in fourth just enough to make you regent health and even if you regen hunger, add a beacon with regen effect.

Idea for the pig one: Fill the entire inventory with carrots on a stick and put a bunch of trapdoors along the sides of the place where the pig walks so that it might fall in because the AI thinks that a trapdoor, opened or closed, is a full block.

I think the jumping torture chamber could improve if there were two people in there. They would have a choice, either fight to the death and whoever wins gets jump boost or stay there forever

I've been looking for torture devices, but after watching your escaping poiseiden vault I've found this. The Hades vault escape was great as well.

I love that it's a wandering trader, and how you got to use a vending machine for the torture room, also if you have a player in the machines, then they could just turn off their master volume in minecraft rendering it useless

Why is he just so dorky and adorable I love it
I've been in a Minecraft maze that was only illuminated by redstone torches. The walls and floors were completely black and nightvision / higher brightness didn't help at all. You had to navigate it entirely by looking at the redstone torches. So everything looked the same and you could easily miss a pathway because you didn't see the redstone torch at its end. That was pure pain and without /tp commands, I would never have gotten out.

I have an idea for torture methods, just make them build insanely complicated redstone killing machine they then have to try :)
This is the most chaotic yet entertaining video ever
Perfect torture machine idea:
You put the victim in a 1 x 1 area, throw poison splash potion by droppers, wait till poison dies out, throw healing splash potions and repeat
For the bell torcher, you could’ve just like add a redstone circuit directly to the bell, don’t need to do some fancy arrows that can miss :)

The vid is great and i love the traps but for the noise machine you can turn of your sounds
To add to the Obsidian Jump torture, you could put a cobweb on the side of the obsidian block that the prisoner will be jumping towards. That way, they’ll have to slowly fall to try again.

Mythrodak really did be setting up the regenerating cobblestone walls for all those prisons he got put in
Idea: tube with cobble generated pillar, you stand on the pillar and have to constantly break the cobble below you or suffocate
watching diorite the whole time on a build is the biggest torture!!

For my money a pumpkin with Curse of Binding is a great torture device. Can't take it off, no durability because pumpkin, and your vision is messed up. Might be completely impossible to remove in a Keep Inventory world
Ways to escape (or stand) each chamber (without just breaking the obsidian and tunneling out, though some of these would be fixed with elder guardians)
The zombie cobweb one: kill the zombie, or with weakness take the spawn eggs out before they get spawned.
The noisemaker one: just mute your volume.
The endless cobble wall: if you get lucky with random ticks, you could very quickly break the glass after mining a block and then you can easily break the redstone afterwards.
The conveyor: you can easily break the slime and honey blocks, which even mining fatigue I think wouldn't fix. Without this, it may be possible to blow up a creeper and mess up the redstone, though i'm not too sure
The parkour: you can either break the wood door (would be fixed with an iron one) and use it to boost yourself up, and alternatively you can dip your toes in the lava and damage boost your way up.
Also, the pig chamber could work if you filled the prisoner's inventory with curse of binding carrot sticks, and possibly you could make the ceiling 1.5 blocks so the prisoner can't get off the pig or they'll suffocate.
