    Thanks as always, an interesting subject which is not often mentioned. What I wanted to ask is if you have such occurrences of deaths on board, you would be in breach of your safe manning certificate, would you get crew replacements before entry to port? or proceed and crew will join once alongside? In the case of deaths in enclosed spaces you would keep a record of the permits and how they were followed, would you retake a sample of the air in the space? and keep the instrumentation sealed off to make sure that they are tested?

      George, thanks for the feedback:) To answer your questions:
      1) Yes, you are right. Under these extreme situations, the Flag State does allow an exemption from the safe manning provided arrangements are made for the person to replaced as soon as practicable. The ship owners will have to arrange for the reliever at the next port. However, sometimes, the C/O has a Master ticket and 2/0 has a chief officer ticket, and along with the third officer the safe manning requirement is fulfilled.
      2) Great questions. You would have to document and provide evidence of the reading on the oxygen analyser and the explosimeter, together with permits and how they were obtained. The relevant entries should be made in the log books and counter signed by Senior officers on board to demonstrate that all precautions were observed.
      3) Basically, if you can provide evidence of the use of ISM checklists and the actions taken, then you are safe (the court of law will view 'negligence' seriously but if a death still occurs after all the necessary action, the court may view it lightly??)

      Thanks for that. As I had a similar question when assisting a mayday and you take people on board more then you are allowed to carry, so with the same reasoning you would get an exemption from flag, as you are obliged by solas to give assistance.
      With regards to enclosed spaces I think it’s the one permit which is most difficult to risk check for as there are so many conditions that can go bad, such as mud releasing gas, inefficient ventilation etc.
      Thanks for the videos once again but even more tank full for the time taken to answer my questions.
