You need to watch Gomorrah La Serie to see how this all started in Naples. Its not the same in Milan, in Milan they all stay under the radar and there are agreements between the bosses that its better to not be an idiot and going around making a scene. Its simple for these young guys in their heads, they want to be a boss and even if they are a boss for a short while before they are murdered they consider this a great achievement and maybe if they where a liked boss they will get a shrine. So for the Milan bosses its about money and staying out of jail and the Naples bosses its about wearing the crown no matter the cost...
