  • Dr. Ebraheim’s educational animated video describes the AC joint injuries and Classifications, how to diagnose it, and the treatment options in a simple and easy way.
The AC joint is stabilized by a joint capsule and ligaments. The AC ligament controls the AP translation (horizontal stability). The Cc ligament controls the vertical stability. Two separate coracoclavicular ligaments that stabilize the distal clavicle. These ligaments are the primary stabilizers to superior or vertical migration of the distal clavicle. The conoid is medial (inserts about 4.5 cm from the end of the clavicle).
These injuries are classified according to the degree of the injury, the amount and the direction of displacement into six types.
Type I: Sprain - pain at the point of the shoulder.
Type II: AC ligament ruptures and there is partial displacement of the AC joint (CC ligament is intact).
Type III: The AC and CC ligaments are ruptured with superior displacement of the clavicle up to 100% of the clavicle width. There is an increase in the CC ligament distance and the deformity is reducible.
Type IV: The ligaments are ruptured and the AC joint is dislocated and displaced posterior through the trapezius muscle. Axillary view x-rays will show this posterior displacement very well. In the axillary view, the anterior aspect of the clavicle and the acromion should align.
Type V: greater than 100% displacement of the clavicle superiorly. Marked increase in the CC distance and the deformity is not reducible. There will be marked gross disparity between both the injured and normal shoulder.
Type VI: inferior displacement of the clavicle. This type is rare. The distal clavicle lies under the acromion and the coracoid.
Injuries to the AC joint are sometimes apparent due to the pain and deformity. Injury results from direct trauma such as a direct fall on the point of the shoulder and if the distal clavicle is prominent, then the patient probably has a high grade injury. If you can reduce the deformity with manual pressure, this probably means that the clavicle did not button hole through the trapezius or deltoid muscles and this determines if the patient will need surgery or not. Be sure to examine the patient carefully because the neurovascular status may be affected, especially if it is a violent trauma that may cause a brachial plexus injury or scapulothoracic dissociation.
The amount of displacement can often be difficult to assess on plain x-rays. If the deformity is more than 2 cm, then you probably need to do surgery. The idea is that there is a higher risk of complications with surgical treatment. The result of conservative treatment is usually better with a quick recovery and return to work. With Types I, II and Type III the trend is to do conservative treatment. Conservative treatment in Type III injuries is controversial, especially in athletes. Surgery is better with Type IV, V and Type VI injuries.
    Nonoperative Treatment
•Treatment the patient with a sling for about 7 days followed by range of motion exercises
•Patient may develop painful arthritis of the AC joint
•Patient may have residual prominence of the distal clavicle and pain with nonoperative treatment that may last for about 6 months.
You will work on and stabilize the CC interval
•Healing of the ligament will occur with no need for graft augmentation (do CC interval fixation).
•Use suture anchors, buttons placed in the coracoid, or sutures passed around the coracoid (both probably have the same result).
•May do distal clavicle excision and you also add AC joint fixation with a Hook plate to support the CC ligament repair.
•CC ligament fixation in isolation could translate the clavicle anteriorly due to the lack of an anterior AC ligament.
    Chronic Symptomatic Dislocation Treatment
•Do tendon reconstruction if greater than 4 weeks (augmented Weaver-Dunn procedure)
•Weaver-Dunn procedure is a good operation when it is performed acutely.
•It may not be good alone in chronic injuries
•It does not matter which procedure is done, loss of reduction can occur and it usually does not affect the result of the treatment.

Will tell my story to help others. I’m not young, about retirement age here in Australia, and was thrown off a dirt bike in the Blue Mountains, landing on my left shoulder. I’m 120kg, and the road was hard packed clay. This was September 2020. So I had a type 3, complete separation of the AC. I paid for surgery 6 months later - had Dacron threads put in to replace the ligaments and recovered completely without medication nor physiotherapy. The only thing I would change, is to have the surgery earlier, but I had to decide first to forget about Medicare, and ante up $15k to get the surgery done privately and promptly. If I had waited for Medicare, it was at least a 12 month wait. After only 6 months, my clavicle was already tearing up through my shoulder on its way to my ear. The surgery was an immediate and welcome normalisation of my anatomy.

@@rumit9946 something to remember, if you put the clavicle back to its normal position using wires, you need to do it within 6-7 weeks after the injury and the earlier the better. The reason is after this amount of time, the ligaments will die, and if you can get it done right after the accident, the ligaments will actually grow back into place and after 6 months, you can have the wires removed and have a perfect shoulder again.

@@vincenicoara7121 I was actually in the middle of trying to compile that footage to make a mini document. The surgery went well, I opted to get CC Ligament transplant and it held down the collarbone again and they took out some damaged cartilage and a little bit of the bone. I am rebuilding strength and muscle gradually and the collar bone lined up perfect with the shoulder; all in all the surgery went well. I thought the other day what it would be like if I kept with psychical therapy and not have the surgery but I had to much clicking and bone on bone rubbing etc it would have worn each other down to stubs which will be harder operate on 10 years down the line. I took really great care of myself using all natural remedies for healing and kept my diet with minimal sugar and high fruit and greens diet with whey protein. It helps to maintain muscle and heal faster or they assume anyway. Bromelain also helps with healing at a quicker rate. I have been skateboarding lightly since but nothing heavy impact because I really want to heal properly.

Yeah man I have alot of bone rubbing and clicking right now too, totally agree - I dont think therapy will get rid of that. I'm also 26 and I was very active with weightlifting and basketball prior to this injury so I dont want to compromise my athletic life. And honestly... the bump bothers me alot aesthetically as well haha.
How long did you get your surgery after your accident? Mine was about 2 weeks ago so I'm hoping to get my surgery scheduled in the next 2 weeks so they have to option of repairing the existing ligaments too. I'm not sure it matters too much but I'm pretty anxious about not getting it scheduled in time for that option. I live in Canada so they suggested surgery right away and put in buttons to tie it all up instead of a removable plate which was only day surgery, in and out in a few hours. I'm almost 3 weeks into recovery and experiencing obvious shoulder stiffness, but It gets better day by day and when I look at my shoulders in the mirror they look just like before and I can already feel myself getting back to normal. Thanks for the excellent information! I have a 5 year + injury to the AC joint, it's on my to-do list to have an orthopedist address it, once some more dire medical issues have been fully addressed (most importantly, hyperthyroidism, resulting in bone loss).
I'll want to get with the orthopedist to see if PT might be more effective for the pain and loss of ROM. Still, that's why we have specialists, the specialized knowledge and experience. Still, on the triage list, it's way down on the list. There's a lumbar issue that is of greater concern, as there may be neurological deficit present.

The injury was 12 days ago, and I am just now able to do certain things. I used resistant bands yesterday, which hurt like heck, but made me feel better the next morning. I even bench pressed 30-lb dumbbells, yesterday morning, at a decline, flat bench, and incline for 5 sets each of 25 repetitions. Before, I was able to bench 100-lb dumbbells for reps, now it's scary to even think about trying it. The resistant bands are great though. Make sure to fight through the pain. Do isometric holds for 30 second repetitions. Cheers!

I just tore my left AC about a week ago from a bicycle accident. Classified as type 3. My orthopedic surgeon says personally, he would not have surgery but will leave it up to me. Seems better not to do it as more complications are possible the longer your shoulder has to heal. Not to mention you’ll have to go through the recovery process all over again.

Hey guys, I decided to not have the surgery. I was in a sling for 2 days and was pretty much back to normal in 2 weeks. Went to maybe 5 physical therapy sessions. I notice no difference in my activities today. I do have a bump on my shoulder though.
From what I have read about the surgery is that the recovery period is 3-6 months. The pain during the first week is pretty high. Surgery doesn’t even fix the ac joint. They just pull the collar bone down to appear normal. I don’t have time from work to even think of doing this. So, I’ll live with the bump.
      Good luck to you both!

@@cigar1310 Hello there, Today I notice no pain whatsoever. I found that going to physical therapy and following their exercises with elastic bands really helped strengthen various muscles in the shoulder. For me, this helped eliminate pain from unusual movements. Sometimes, I would get a sharp pain if I made a sudden move or a jolt but that too has gone away. Give it some time, do some exercises to strengthen you shoulders and I’m sure you you will get better as well. Cheers!

Though I don’t think landing on my shoulder the past couple of months is the reason behind the discomfort.
Btw I still have a visible bump on my shoulder, and multiple scans have shown that my collar bone have moved 5mm up, which would be the reason for the visible bump.

@@emilschmidt644 Good to hear you can do the activity (goal keeper perhaps)? I have been to the hospital today. It's a tossy 2 or 3. But the bumb is quite large. The doctor said the bumb will go away (I don't believe that when reading other people's experiences). He says that the bumb is "mostly" from swelling. I said that I disagree because he barely put pressure on the bone when checking how much luxation was possible. I told him to put more pressure. He then acknowledged that it was indeed worse than he expected. I asked him many other questions, such as: will the joint heal/reconnect again over time. He answered: "Yes". (maybe he meant it in a different way then I would describe it) However everything I read/learned so far tells me the opposite. He also said to do fysio therapy the coming weeks. And move the arm based on pain. Which is what I will be doing. But I am just really afraid how this plays out in the long term. I have read people getting neck and back pain or even issues with legs due to imbalance of the body. And obviously the loss of strength is a concern. And the "weird" feelings when moving the arm. I hope everything will go back to normal :( At least the pain is more or less gone as long as I leave my arme rested on desk or in the sling. Moving either feels really strange (not sure how to describe it really) or hurts. Good luck with your recovery, but you seem to be doing good relatively speaking 👍

I got myself a type 2 or 3, around 4 years ago(~2018) while trying to bend a steel bar,(so from pulling, not falling or so) i kept my arm close to the body and not use it for 1-2 weeks ,at the doctor, he just push on my deltoid muscles in 3-4 spots, ask me if hurts, i told him no, and that i was concern about the "bump", he pressd on it and no pain, so he just gaved me some pain pills and a creme/gel, to me it looked like he didnt pay so much attention, it took a few months(3-4) untill i got ok with it, after like 2 years it hit me again, probably an overuse of the shoulder, again ive took a few weeks brake, then again in december 2021 , again ive been to the doc. , painkillers/creme,he said to use the pain as indicator of what is to much untill it recover.
( october 2021-april 2022 , i took a brake from my workout, and just restared in may , ((no pushups, sems those are bad for my shoulder now)) So in may 2022 it happend again, guess because ive hit a boxing bag, like..10 times. i gonna get an X ray,.
Now im 26, during the 2 years after the first injury i kept doing body weight exercises at home,no pain, all normal, pushups, pullups, curls, etc, it was fine, but since the 3rd time i felt considerable pain i stop doing shoulder motion moves, i do have a bump on my right shoulder, since the beggings, the colorbone(other end of the clavicule) it also shifted a little, i can see that it feels different then the good clavicule.
-[[[ ...MY ADVICE, if you go tru AC joint issues, take it serious, the shoulder is the most flexible joint of the body, go straight to a docto..or two, get an X ray, take at last 2 months of rest for your shoulder, look to do recovery with a physic teraphist or so, use ur arm, keep it flexible and active, stay in shape, but always remember that is vulnerable to injury]]]
If you have questions, or advices, let my know.

I still have more than a slight bump, probably a 1-2 cm bump, permanently. I think its because the doctor removed the entire acromioclavicular ligament, which was smashed anyway. So there is no longer that connection to stabilise and keep things in line. In general - now 10 months after my surgery my shoulder is still weaker than the other side, but seems okay except for this permanent bump. I sometimes wonder if the surgery made any difference at all or if it would have healed up well (perhaps with less of a bump) on its own. I will never know.

I would've done surgery but cannot as my coracoid process also broke. So nothing to attach the LARS ligament on. Now i need to wait 9 months minimum to be sure and then if i feel the need i can do surgery. So at this stage its a waiting game. I can say however for me this is a looooong process. I'm very active cyclist, mtb and road. Even tho i do stationary i'm still very depress. My shoulder feels like it is taking longer than usual. Out of the sling and can move my arm, but only to some degree. Still very weak and i put no weight on it.

I'm two years in from grade three. I'm having surgery next week. It got better for awhile before it got worse. At this point I feel surgery is worth a shot before I accept I'm screwed up for life. The bump still sticks way up and I have pain everyday. Especially when it gets cold out. I got mine from being thrown from a motorcycle and landed on my shoulder. But hey at least I didn't die. Hope you have good luck. I would get the surgery before arthritis kicks in. Why wait till then it will only make it worse. Wish I would have fixed it right away.

Sean Anthea or Otis, do you have an update? I’m getting close to my surgery date and having second thoughts.
How has it gone for you guys? I hope your recovering well.

Sunday Morning my surgery date is March 18th, I also feel like you and worrying but it’s what I’ve wanted since the accident. I’m 24 so I feel like if I get the surgery now and suffer for another 12 months recovering, at least that’s better than chronic arthritis in a few years and have to deal with that as well as the joint still being unstable.
How long since you injured yours? It’s still hard to accept now as I played rugby, boxing and heavy weights. I just want to be able to play sport again with it moving around as much as it does.
