Personally i feel pg should be first choice as UPSC exam u can clear after completing ur pg also, plus after finishing pg and senior residency better apply for post of consultant rather than medical officer

You did right thing even i cleared upsc (got railways) but didnt join instead joined pg in OBG (Safdarjung hosp, Delhi). What u can do now is look for specialist post(interview basis) Same upsc post u cant join, you need to write upsc (mo ) exam again, since form has come fill the form get ur admit card u can give now, clear join

Medical certificates for sweepers etc.. you'll also become a health inspector for restaurants, a prestigious post... Even 5 star hotels will bow to you.. . the payscale is the highest of gdmo posts in all of India.. rs. 1 lakh+
