I don't know exactly but the story of the game is that the guy killed his wife and kid after drinking hence the beer bottles on the table and is hallucinating. That or he got drunk and then killed them. Possibly with rat poison which explains the rats

That's a line from Dante's inferno and there's a song someone made based off that book called Charlie's inferno so I just thought that was funny lol.
