But nope, the intended sound was the Item Obtained sound, which only played for two trainers. Turns out it hadn't played at all! That sound that played for those two trainers was the Level Up sound.

Haha! I was extremely interested in the gym badge sound effect oversights so I looked into it a bit more. So first off, the sound effects are being played through text commands that are a part of the text strings that are being printed on the screen. Text command $11 is used by the overworld to play the "got key item" sound effect (got Oak's Parcel, Coin Case, etc.) As you mentioned, that particular sound effect is not accessible during battles, because the battle's copy of the music engine has a slightly different list of sounds effects than the other two copies of the music engine. And due to how the sound effect IDs aren't really IDs, but are instead an offset from the beginning of a list of every sound channel, the "ID" for the "got key item" sound effect ends up pointing to the second of two channels that belong to the "poof" sound effect in the battle's music engine (the "poof" sound effect plays when you send out a mon at the beginning of a battle or when you throw a Pokeball) This is the command used by Misty, Sabrina, and Blaine. So the developers intended on the badges being treated as key items, at least in the case of these three gyms. However, there is a more subtle mistake that you did not mention. Text command $0B is used in the overworld when you pick up an item ball. But again, the "picked up item ball" sound effect is not in the battle music engine, only in the other two music engines. By luck, the "picked up item ball" sound effect "ID" ends up referring to the "level up" sound effect (a sound effect that is only in the battle music engine and is not present in the other two music engines) and that is what you hear in your video. Not the "got item" sound effect.

Considering the only Pokemon where this happens is the one with an index number of 0, I would not be surprised if it were related with the way the game checks a wild battle is over. One part says: "Look, there's no pokemon on the opponent's side... I only see a 0. Load the map tiles!" just as another says: "But wait! I am waiting the Pokemon 0 to leave... and I'm still seeing one! The battle is not over!", hence the broken tiles and the on-going battle. Ditto may happen for the same reason Transforming Pokemon are immediately turned into Dittos.
I don't know, but that gives me an idea for a story. "'M is guarding the Ditto behind it, but when 'M is captured, the Ditto is revealed to be behind him." ...insert random lavender town music here
Here's another odd one for you to try. If you go into Surge's gym and check the bottom-left most trash can, a sprite appears in the bottom left of the screen.
I though 1:30 was a thing that was done on purpose, or at least left in because it sounds cool and appropriate... Interesting to find out it was actually a glitch.

I am pretty shure you already know it, but there is a little glithc with professor oak when he gives you the starter. When you repeatedly try to leave at one point some collums of letters appear on the screen. Scykoh showed this in his Pokemon Glitch Quest series

How about the glitched shadow when using escape rope/dig in some areas... for years I wondered what I did to cause that. Then I find out it's always like that. Seriously? +ssj99lucario It corrupts the box if it's Level 0, and has 3 variations: a "Missingno", a glitchy Slowpoke that crashes the game if encountered, and Charizard 'M, which brings the poor game to its knees and beats it with a sledgehammer. For example, Charizard 'M can turn 5/6 of your Pokémon into Bulbasaur that almost always disobey you, and explode. It can also do stupid shit like turn Mewtwos into Caterpie... while turning other Pokémon into Charizard 'Ms. I'm serious. The Bulbasaur thing (goo.gl/ylwfN4) and the Caterpie thing (goo.gl/pBIrY7) are legit (Q is Yellow's version of Charizard 'M, and Q's easier to get than M so most people just use Q).
+PinkCharmander Technically any pokemon caught at level 0 will cause the PC to freeze trying to withdraw. The reason its associated with 'M is because 'M is the only Glitch pokemon obtainable at that level.
+Sauzels the frame actually has some uncanny similarities to the one from the titular "minigame" in the Japan-only DS game "Nanashi no Game". Look it up.
