Door on a second floor leading to nowhere... I asked that at a house-turned-museum once. In this case, it led to the roof of the porch. The guy said it was so that the maid could shake the rugs out there. Not sure why a prison would need something like that?

The door is either an old delivery port or a destroyed balcony. Most likely a delivery port for heavy goods.
