Without the "an" article, it would only be (rarely) used in response to a direct question. Which pie did he eat? ... He ate apple. Even then, it's a really rare and odd response. (But even then, you're more likely to say: he had the apple (flavour). ...

In English nouns can be either countable or uncountable. Apples are usually countable, but in certain situations "apple" can be used as an uncountable noun. I agree with you that this would not be a common situation in most normal circumstances, but grammatically that is the case.
