You can get custom G&Ls through Sweetwater! Just call up a Sales Engineer and they can get your ideas down, then get back to you with a price. I'm planning on doing it myself. I have an idea for an ultra-minimalistic SB-1 with no knobs and no pickguard. Just one, three-way switch that has full volume/full tone on one position, 70% volume/full tone in the middle, and 70% volume/rolled off tone in the third position. Ebony fretboard with no inlays (side-dots only). Huge flake paint with matching headstock (not sure of color yet). Going to experiment with the electronics of that myself by mocking it up in a LB-100 Tribute before I get the spec nailed down.
It's actually not a crazy amount more, but it all depends on what kinds of woods you choose, what type of construction choices you make, and how wild you want to get with it.

Having seen the prices for non-custom Fodera basses, I'm guessing a custom version will cost much more.
