    THANK YOU SO MUCH! I have always struggled with this and had to use a table on my cheatsheet to determine the axis when I read EKG. Now, I finally understand and no longer need to look it up.

    In conlusion
    If lead 1 positive look at lead 2 - if thats positive then its a normal axis
    If lead 1 positive look at lead 2 - if thats negative then its a left axis deviation
    If lead 1 is negative look at aVF - if thats positive then its a right axis deviation
    If lead 1 is negative look at aVF - if thats negative then its North west deviation

    Good grief. Why are the explanations online screwing around with angles and circles, when you are going to find the axis information by looking at the overall signage of the relevant ECG graphs? Thank you so much for not letting the geometry complicate things.

    Do the length of the R and S spikes have to do with determining the cardiac axis?
    I'm reading a book that says that when they're equal in a lead then that lead is at a right angle to the cardiac axis. But the book is really difficult and I'm not sure I understand.

    Simpler is to just look at I and aVF - you can tell left axis deviation by them as well

    My EKG book by Karen Ellis says that you have to look at Lead 1 and avF to determine the axis quadrant. I get confused! No Lead 1 and Lead 11 for normal and left axis deviation. It uses only L1 and avF

    I thought that a RBBB by itself already gives a right axis deviation. How can you know that there is also a posterior fasicle block?
