  • This week’s UCSF Department of Medicine Covid-19 Grand Rounds features three segments. First, George Rutherford will review the state of the pandemic, including the implications of hot spots in the United States and Europe. Annie Luetkemeyer then gives us an update on outpatient treatments for Covid, including monoclonal antibodies and a large group of oral agents (such as ivermectin, vitamin D, fluoxetine, colchicine, and hydroxychloroquine) that have been touted as beneficial despite uncertain evidence. Finally, Adam Lauring of the University of Michigan will describe emerging data on Covid variants, including the current uptick in cases in Michigan. The session is moderated by UCSF Department of Medicine Chair Bob Wachter.

    I'm glad you're having a discussion but also we need to consider what other countries are using as far as Therapeutics and some of them have had incredible results worth a thought

    The poorly designed ivermectin study in JAMA failed to give statistically valid results because it was underpowered. Even the JAMA editors are now concerned about that study. Can't believe this was even brought up. There have also been many good studies, not to mention the entire country of Peru, demonstrating that ivermectin is very safe and effective. Why didn't you cite Dr. Tess Lawrie's meta-analysis? Dreadfully biased review.

      Apparently, the Columbian RCT participants were informed that their trial consisted of a certain "molecule" (or placebo). Chances were good that sick patients availed themselves of OTC Ivermectin. Both arms were doing remarkably well. Goalposts had to be changed.
      There is more to be interested in than rna dependent rna polymerase.
      The world needs Ivermectin while we wait for Merck and Pfizer to undergo trials.

      Not sure where to ask this but the numbers become very confusing in how we responded as a country when it’s obvious there have been political influence on testing and data collection. The CDC grouped cv19 with flu and pneumonia in July 2020, labs clearly over reported false positives, PCR testing has been hinted at not being designed for this and WHO changed testing protocol the day after an inauguration. So, it’s hard to trust the numbers, as well as the response to the numbers. None of this includes the many many deaths that occurred in people with major co morbidity. How does your studies account for these issues? Is there a procedure for acknowledging these major issues ?

      @Kicapu Is that why my life long best friend who was given three years to live before colon cancer took him in 2010 that survived covid in January 2020 and passed in January 2021 has covid as a "comorbidity" of his death certificate?
      His wife and kids want to know why as do I and my family.

    Thank you for continuing efforts to keep us informed. One strong recommendation: most of these slides are far too busy to appreciate their content and presenters should design their content to clearly communicate information (not to densely pack-in information). This has been a challenge to this series of talks over the past year. Presenters need to consider what the audience sees and not what they see on their own computer monitors. Also, the moderator could make better use of the screen space: I don’t need to see the presenter when they are discussing their slides; I need to see the content of their slides. Thank you for considering and for your continuing efforts.

    In the UK the outbreak of the B.1.1.7 variant was not controlled by vaccines, but by lockdown. The peak in cases was 9th Jan 2021. Vaccinations did not start until 14th Dec. Requiring at least 2 weeks (and 3 weeks for the elderly, who were the first to be vaccinated). The impact of vaccines in controlling the outbreak was miniscule.
    The immunity resulting from the vaccines, has slowly made an increasing contribution in maintaining the reduction in cases. The cases are now rising, the underlying trends have been upwards for weeks.
    The third wave in Europe is now underway.
    For the US it is just starting. The trip point seems to be about 10% prevalence.

    Please share the data that clearly shows that vaccines are responsible for affecting the statistics, versus lower infection rates due to prior exposure and reduced testing. I feel it is presumptive to say it is solely driven by vaccinations.

    The most basic question on variants unanswered. What are the biological differences of these variants biologically in people? To throw terms like more transmissible without clinical studies makes these pronouncements useless. Do make science and not guesses as you do when you mention statistics and not biology or clinical studies. It's hard to do science but people need science not hypotheses. If one need guesses there is more than enough from CNN MS etc, they're not so good so don't join them. I know science standards under Covid plummeted but the opposite is what we need.

    I'm thinking of Professor Vincent Racaniello at Columbia University, who is very adamant about defining "strain" as having distinct biological feature(s) in comparison to the WT.

    George, for all my respect for you, I think you are already clearly outdated. We have a clear indication of the fourth wave in the US which is not only Northeast and Midwest but wider as well, rising in some Southern states as well. Of course B.1.1.7 will dominate, it does this in Europe, we can see it. But there is enough data from Brazil to give us a great pause about P.1. Not only that it is faster than B.1.1.7, at least in Brazil, it seems to be as lethal as well. So in the long run when it takes hold we shall see P.1 as our biggest problem, in my view. To deny that these variants are more dangerous is truly foolish, we have enough of data to know that this is the case. I am really getting tired of virologists pretending to know more about Epidemiology than Epidemiologists. For one thing, Epidemiologists know some math and I am yet to find a single virologist to show the most basic math knowledge. Isn't math a requirement in US universities to study Biology? If it is then why do these people think that this is something to be proud of - not knowing any mathematics? Racaniello and his clique seem to be totally incapable of calculating simple ratios and yet they seem to be always ready to comment on models and probabilities. With a consequence of making utterly foolish predictions on almost every turn. How can anybody call himself a scientist without even a modicum of math knowledge is a complete mystery to me.

      She did and of course rubbished it. Obviously under instruction to do so. Quoting unsubstantiated claims again. There is not one person who has been harmed by Ivermectin. Four billion doses over 40 years for various parasitic diseases such as river blindness is proof positive of the safety. They talk about people overdosing on veterinarian Ivermectin without a shred of evidence. If it were true they would obviously give the details. They don't. Try yourself to find it online. It does not exist. No names or anything. Repurposed drugs are a no no for these people. Pharma can not make money on repurposed drugs so they and their cronies rubbish them. It's an absolute disgrace and people are dying as a result.

    I had an allergic reaction 7 minutes post jab. Pfizer. first dose. Flushing, disorientation and diarrhea. Please start talking about these reactions.

      small: hopefully you were observed as the CDC recommends, and they responded to you effectively. Your reactions, and similar reactions, are what clinicians in facilities doing vaccination are expecting and prepared to address. Speak with your PCP regarding your second dose.

      I agree. Anyone wanting to learn more about it, visit flccc website. They have videos and videos of its efficacy.
      The folks in charge wouldn't be able to get their vaccine pushed under EUA with this info out in the public sphere. Be careful and do your research; connect the dots; put the puzzle pieces together. No need for the fear and panic.

      This is true. Also Germany was relatively lightly hit during the first wave. But both France and Italy were badly hit by the first wave and are once again hit badly.

      The 3rd wave in Europe is being driven by the B.1.1.7 variant becoming dominant. When the prevalence of the variant reached 10% in the UK the cases accelerated rapidly, the doubling rate was down to 10 days. In one month the deads doubled and we lost 50,000 additional lives (the equivalent of 250,000 in the US).
      Despite having achieved over 50% immunity (by natural infection and vaccination) and being in strict lockdown since 26th Dec, the cases in UK are beginning to rise.
      As now they are in the US.
      The prevalence of the B.1.1.7 variant in the US is still relatively low, just under 10% in the states with the mosts cases (Florida, Michigan , etc). I think that the US could suffer a 3rd wave similar to that in the UK and some other European countries (Spain, Ireland and Portugal) last year, and now most of the other countries in Europe.
