you can do it without any sort of ghost item at all if you have prayer and he gets to 20 HP, you clearly did it here, and if you pray at 20 HP or lower Hatred just instantly dies, Based on my speedrun and even casual playthroughs that can get this in about 4-5 turns, you can totally kill him before he even gets to unleash his charge attack
I have a theory that The Kitchen Wizard actually knows what happened to him and who’s responsible for it so he’s pretending to not know who Terry is. He might be a future antagonist. I think the fact that he was gonna wrap up the sword for the player was meant to throw us off.

unfortunately not, with the damage multiplayer on hellmode you'll get KO'd by his nuke (anything above 200 damage KOs you and his nuke deals 240 damage)
Very possible without Gusto and Hard Mode + Double Pain, use Power Swings with Knight and Ante Up until you can one shot the boss with Pray. Use Resurrect if you’re doing it with Hard Mode or Double Pain to stay alive, you’ll still have enough damage to Pray
there isn't really a definitive name for using all the hardmode cards. some consider this without thick skull hellmode, some don't, and some just calls it hardmode+ now me personally i consider this hellmode

You can block the explosion and while your health slowly drops just heal but you gotta be fast cause it drops over time even when picking an option
