Hey Nio! Been a while. Today my story's going to be on the crash at 3:04. (INVESTIGATION INCLUDED)
Orange 1739 | 14th November, 2018
Following previous crashes due to pilot error becoming more common, Orange Airways has updated their standards to become a crew.
A flight called Orange 1739 is boarding at Crystaline International Airport heading to Azara. This route is the most difficult, especially during landing at Azara Airport. Said route is a new route just introduced 10th October, 2018. This plane is a 5 year old 777. The length of the flight is estimated to be 6 hours.
The approach consists of a fast descent to the runway because a hill is in the way. This airport commonly has fog, making it more complicated combined to the approach.
Captain flying the plane is 54 year old Michael Green. He was an experienced pilot in the airline and has 4500 flight hours on the 777. First officer or co-pilot is 35 year old Whitney Stevens. He was also an experienced pilot having 2500 flight hours on the said plane. Both pilots are new to said route.
At 12:20PM, the plane pushbacks and taxied to Runway 28L. On 12:26PM, the plane takes off and climbs to 35000 feet. The climb is as expected and on 12:35PM the plane reaches cruising altitude. The pilots are chit-chatting about their future approach to Azara Airport. In-flight services commences.
As the plane nearing it's end of flight, the plane descends slowly to the runway. Both pilots is focusing on the approach. The time was 5:58PM, where the pilots are required to see the runway. The pilots are confused because they didn't see the runway. Unbeknownst to them, the plane is too high to make a landing.
They continued descent, and then they saw the atc tower to their right hand side. Both pilots are confused, as they look out to their side. The result of being confused, the first officer tried to increase thrust, but instead lowered the thrust. Captain looks back up front and noticed the first officer making the mistake. The captain tries to increase the thrust while both pilots pulls back the yoke, but it's too late. The plane crashes in front of the runway. Out of the 161 people on board, 142 survived. Both pilots are killed.
INVESTIGATION PART (Read this to find the cause of the crash)
Investigation commenced after 2 hours of the plane crashing. An investigator called "Smith Jacobs" started the investigation by investigating the ATC at Azara Airport.
Smith: Hello, I'm an investigator looking into the crash of Orange Airways Flight 1739. I understand you were working in the tower on the day of the crash. Can you tell me what happened?
ATC: Of course. As the air traffic controller on duty that day, I had the responsibility to monitor every plane in the area, including Orange Airways Flight 1739. Our tower has radar technology that allows us to track the movement of every plane in our airspace.
Smith: Can you walk me through the conversation you had with the pilots?
ATC: Yes, the pilots made contact with us to confirm their approach to the runway, and I gave them the necessary instructions. I tracked their progress using the radar and could see that they were deviating from the prescribed approach path.
Smith: Can you tell me what they said?
ATC: They said that they were having trouble seeing the runway and that their altitude was too high. But for me they were just fine?
Smith: Did they follow your instructions?
ATC: Of course. As the air traffic controller on duty that day, I had the responsibility to monitor every plane in the area, including Orange Airways Flight 1739.
Smith: Can you walk me through the conversation you had with the pilots?

ATC: They said that they were having trouble seeing the runway and that their altitude was too high. I was pretty confused since it showed a proper descent towards the runway. They appeared to be confused.
Smith: Did you have any contact with the passengers or any other witnesses to the crash?
ATC: No, I did not have any direct contact with the passengers or witnesses to the crash. However, I did communicate with emergency services immediately following the crash to initiate rescue efforts.
Smith: Thank you for your time. Is there anything else you'd like to add?
ATC: No, that's all I remember.
Investigator Smith arrived at the crash site with his colleagues to inspect the plane and gather evidence. They approached the wreckage and were greeted by heavy smoke and the smell of burnt fuel. The plane was lying on its back, and the front half of the fuselage was severely damaged.
Smith: (to his colleague) This is a mess. What do you think happened here?
Colleague: (after surveying the wreckage) It looks like they came in at too steep of an angle. They must have lost control of the plane.
Smith: That could be it. And the pilot's communication with the tower? Were they able to get that from the recorder?
Colleague: (nods) The pilot was in contact with the tower, but they were having trouble seeing the runway and were too high to make a safe landing. Looks like they tried to correct, but it was too late.
Smith: (nodding) That makes sense. Let's get to work here. We have a lot of work to do.
They both began inspecting the plane, taking detailed photographs and examining the wreckage for further clues. They worked efficiently and without emotion, knowing that their findings would be critical to understanding the cause of the crash.
They then found the blackbox and bought it to the investigation room. They then played the audio while taking notes.
Captain: Orange 1739, we're approaching runway 28L, requesting clearance for landing.
ATC: Positive, orange 1739 proceed landing.
Captain: Clear to land, 1739
First officer: Wait, our altitude is too high?
Captain: I see. Hmmmmmm. 1739 contacting Azara tower, confirm our altitude is 1000?
ATC: Negative, your altitude is 100 according to the radar.
(Smith pauses the audio)
Smith: The plane says the altitude is 1000 feet but the ATC says 100?
William: You have the FDR right? Give me it.
William: Uhh, it says here its 1000.
Smith: That means the ATC is wrong?
William: Positive
Smith: But why? A faulty radar? The last time i investigated the ATC the radar is fine? The plane maintenance records also shows no errors?
William: It may be a miscommunication right?
Smith: I mean, it was a busy day as reported by the ATC. The ATC possibly misread 1000 as 100?
William: Let's continue the audio
Smith: (plays audio)
Captain: But we should have landed by now?
First officer: I don't really know
Captain: YOU LOWERED THE THRUST NO
***Terrain terrain pull up! terrain terrain pull up!***
Both: PULL UP PULL UP PULL UP COME ON PULL U-
(Audio stops)
Smith: Let me visit the ATC tower to see the flight's history
Smith went to the ATC tower
Smith: Hi again, I want to see flight 1739's flight history.
ATC: Sure
Smith:...It says here during descent it's 1000 feet?
ATC: Yes?
Smith: Why did you say 100 though?
ATC: Ummmmmmm, uhhhhhhh, accidentally?
Smith: That's the cause of the crash!
Smith returns to investigation
Smith: I contacted the ATC tower and the result is miscommunication, and pilot error.
END OF INVESTIGATION
The cause of the crash is miscommunication between the ATC and pilots combined with the weather and pilot error. The ATC that is responsible for the crash is fired, and the airline updated training for ATC and pilots. The airline also requires the ATC to confirm the altitude that is read out.

On January 6 2016, Asiana airlines 967 (ASA967) was a Boeing 777-200ER going from SFO to ICN. The flight crashed into the pacific ocean midflight because of this: Last Year, 2015 This 777-200ER was deliverd to Asiana airlines and registered as HL7745. On its first flight fron ICN to SFO, the pilot which was a rookie pilot landed at SFO with a huge tailstrike, causing it to be repaird for a year. When it was back on service, the plane was controlled by skilled pilots but sadly plane lost its tail and crashed.
