The anime is Madoka Magica (or if you really want to get anal about it, Puella Magi Madoka Magica). *Spoilers ahead: Read at your own risk* Sayaka turns into a witch in multiple time-lines, Mami loses her head (literally) in multiple timelines, Madoka becomes god in the final timeline she is featured in (turns into a witch in all the others and forgotten by all except Homura and her little brother, Tatsuya, in the final timeline (that's mentioned in the anime series)), Kyoko dies in a handful of timelines, and Homura somehow survives every single stupid timeline. There is only 1 season with 12 episodes, but there are about 3 movies~
