Park Shin Hye and Lee Sung Kyung used to work together as first and second female lead in another medical genre drama as well, may be that's why they asked her to appear in this medical drama too.

i think sung kyung made a cameo in doctor slump bc of her camaraderie with shin hye since they have worked together before in the kdrama "doctors". doctor slump had been trending on netflix since the early episodes came out, so it doesn't need anyone to boost their viewing.

Lol. Lee Sung Kyung made a came because she is closed friends with Shin Hye after they filmed the Doctors and the director of Doctor Slump is the same director as of Weightlifting Fairy Kim Bok Joo where LSK was the female lead. Also she has previously worked with Park Hyung Sik too when they voiced the movie Trolls in SK. But the ratings are doing well even without LSK.
If they really wanted to use cameos just simply to boost ratings, they would have asked Lee Min Ho and/or Kim Woo Bin to cameo -- that would have drawn more fans in since the two leads previously worked together in the Heirs.
But the drama is really popular, not just in korea but overseas too it's been constantly in the top 10 of Netflix in several countries since it started.

Park Shinhye and Park Hyunsik are already top actors where any drama they’re in, especially together would do well, and it was just a nice cameo for Lee Sungkyung to join in, her appearing wouldn’t boost the drama at all because it’s already doing well
