Current Concepts in Anaesthesia for Shoulder Surgery say that patients may be awake or asleep, sitting or lateral and have systemic, peri-neural or intra-articular analgesia, or indeed or a combination of all three. (see www.ncbi.nlm.nih.gov/pmc/articles/PMC3785039/)
In this case the patient was awake.

DarkMagicianGirl nope. A special kind of medicine called "anaesthesia" is always used. This puts the patient into a special sleep in which they cannot feel anything. They also can't wake up until another type of medicine is given to them. They don't even dream!

They used an ultrasound by pretty much running it down the length of the LEFT deltoid, and get a higher quality image than an MRI scan.
Of the RIGHT scapula.
Viewed posteriorly (you can clearly see the scapular spine, the torn supraspinatus tendon, and the intact infraspinatus).
I know it's just a flash game but I couldn't help but chuckle at this.
