Well since there can only be two, no matter the time frame. So lets say 10yrs go by, a Sith Apprentice would eventually become a master / darth, but not thee master. Unless their master falls, be it by someone else or their own hands. Darth Maul, Count Dooku and later Darth Vader, were all Sith Lords earning the mantle of Darth. They just still all anwsered to Palpatine.
A dark Lord of the Sith is not just a master, it is the equivalent of a Jedi grandmaster. A Sith Lord could be equal to a Jedi knight, who can be masters once they take on a padawan.
Reaching that rank especially in a society where social darwinism is king, is certainly in a large part thanks to their already immense power and finnesse in combat. Maul was trained for his entire life by Palpatine, Tyranus was already one of the best jedi of the order prior to becoming a sith, and the same goes for Vader

for Lucas and his movies 2 Darth were ok, but as soon you invent Bane and the Rule of Two you have to implement regulations for the ages prior to Bane. dunno if LucasArts sat down with Bioware for KotoR which by its preset of Old Republic era takes place millenials prior to movie era and Bane to solve such an issue. Bioware's SW:TOR mmo is fine with multiple sith and lords and darth and mored Darth than the 12 members of Dark Council existed.
It would have been nice to see an alternate ending where Anakin reached his full potential as a Sith lord he would have overthrown Darth Sidious easily.

Since Palpatine didn't want Plagueis finding out that he was training Maul as his apprentice, he basically made it seem like to him that he was just an assassin. However, he still gave him the title "Darth" behind Plagueis's back!
In legends they did fight eachother. Well a clone of Maul. He beat Vader in the physical aspect but Vader used his wits in a split second Maul got distracted by some Volcanic activity in the distance. Vader suprised Maul by stabbing his lightsaber through his own robotic body.
