    Does a partial ACL tear need a surgery?
    You typically suffer a torn ACL landing from a jump or planting your foot to quickly change directions. The knee buckles, and you fall to the ground. You feel (and sometime hear) a pop in your knee, and you develop intense pain and swelling.
    Usually you suffer a complete ACL injury, where the entire ligament is torn. If your doctor orders an MRI, it might show a partial ACL tear, but only a small number of fibers are intact. Often it reads high-grade partial ACL tear or something similar. This is essentially a complete ACL tear in the sense that the knee is still unstable, and the few remaining fibers cannot stabilize the knee.
    In this video, I discuss the distinction between a complete and partial ACL tear. I explain when you could need surgery if you have a partial ACL and how the doctor can make the decision.

      Hi David, thanks for this helpful video. I have got a partial ACL (Grade 1 what the MRI reports say) tear, few doctors said I may not need surgery if i continue therapy or certain execrcise. But few doctors have suggested o for surgery bcoz my knee is still in an unstable position & I can't stand for even few minutes. So plz suggest what should I do now ?

      I have done an MRI scan on my right knee, there is an explanation that my ACL was partially tear, but after being examined by an orthopedic doctor, my ACL completely tear or totally rupture, which one should I believe?
      this incident has passed a month, and my knee is healthy now, and it feels stable for walking and doing some regular activities (exclude sports), do I need surgery?, what are the long-term effects on my knee, will calcification or osteoarthritis occur?

    My doctor recommended no surgery because I was 47 years old. I listened. Now, 3 and a half years later I have so much cartilage damage I have terrible pain. It is nearly impossible to use stairs. I had to reduce my work hours by half. Knowing what I know today this is my advice: If you have pain, swelling, or your knee is buckling insist on more treatment. Do not listen to lawyers who what a fast close. Do not put weight on a sore knee. Do not push yourself. You will pay dearly later

      Thank you for your advice. I’m 46 and completely tore my ACL and the first dr said because I’m not an athlete and due to my age that dr wouldn’t do surgery. So I’m seeing another dr and this dr said I’m a good candidate for surgery.

      @@angiefinney132 Right on. I am not going to have surgery. But, I am proactive to not stress my knees. I do exercises from therapy, wear a brace. Take care. You deserve to get the best treatment available

      I know this is a old comment but I tore partially tore my acl most of my acl is still good. Doctor said come back in 3 months and I’ll check u out again 🤦‍♂️Check out bpc157 I’ve been on it for 2 weeks and I can jump and cut angles no pain my knee is vary stable and getting better.

    i have a partial tear she said id be fine if i did physical therapy. But by the look of these comments everyone who had a partial tear just tore the whole thing.

    Been living with a partially torn ACL for 2 years, having surgery next year as although I can get by day to day no problem, I can't play football or ski which are two real passions.. can't wait to get it sorted!

      i’m not really sure what i did to mine i was playing basketball and it just popped i sat out for a minute but when i jumped again it popped again and i just fell it’s been hurting for 4 months now

    Hi De. Geier,
    I had a ACL lesion and on the MRI immediate diagnoses it said it was a severe partial/total tear but when I went to see the specialist one week later he examined my knee and said he didn’t believe it is the case for surgery; he referred me to 6 weeks of rehab and then decide, cause the damage cause de mri to be blur. He seemed very confident, even though I told him I live a very active life. I may look for a second opinion.

    I have been managing a pacial ACL since over 12 years now I do everything but when it comes to physical activities like fight running fast am always scared it has really affected my confidence and mural I over look many people because of my knee I use to be a strong guy but my knee took my strength away that's my stroy

    Im 25, I partially tore both my acls playing basketball coming down from a rebound. Didn’t do surgery but the pain that im dealing with is crazy. Its been close to 2 years now and i started to strength my legs and its starting to make climbing stairs and just walking alot easier. Before these last couple months i used to have to basically crawl up the stairs and going down was a even bigger struggle. Hopefully by strengthening the knee i will keep improving.

      Same I'm 19 and had a partial acl tear 2 years ago while I was playing volleyball. My knees r better now but it is still vulnerable so i need to be very cautious with my physical actions as too much pressure on my partially torn acl always gives me a very bad pain. Im hoping to get a surgery in this 1-2 years as with my current condition I know that it'll just be worst in prolonged years

    I'm 28, and sustained a significant ACL tear over a year ago, but maintained strength and 100% use of my knee without surgery, against the advice of my surgeon who advised that although usable, it would become injured worse in the future. I recently re-injured the same knee from ?hyperextending? it playing volleyball. Probably getting surgery the end of the month using an autograft.

      I appreciate this was a long time ago but how did it go? Have you had surgery? I've pretty much done the same thing, injured my knee 3 years ago thought everything was fine, looked like it was resolved and I just done the same thing. MRI shows tears to ACL and meniscus, and will need surgery.

    I have a partial acl tear and one of my bundles is intact while the other is torn. I have half an acl and my knee feels stable but I’m worried about the consequences in the long run.

      @Alexis Quiroz for me it’s going on 2 months. Since they told me MRI came back showing partial ACL tear. And when I’m not walking it doesn’t bother me that much. But i walk on average 3-5 miles a day at work. So by the end of the day my knee hurts. Therapy hasn’t seemed to help me yet.

      Mine has been torn going on 3 years and it’s pain as a partial tear I feel horrible it’s hurts all year around buckles back I’m thinking about getting surgery because when I get older I’m going to have issues and I’m only 20 going on 21 this year this happened in high school in my 10th grade going to 11th

      it depends on how you feel the knee if you can do sports without feeling instability on the knee, then it's ok, with the cruciate ligament there are 2 categories of people: those who have no instability after partial rupture, but do not know exactly what are the chances to break it completely and those who have episodes of instability after partial rupture

      @@razvan6447 I am able to walk and kinda get back into running ao my life is getting back to normal except with not soccer. I tried soccer and I am able to run, kick the ball, and push but I had the problem that in one moment like at the end my knee had a sensation and I had to get out. It felt like a crab kinda but worst ig not as bad as when I got hurt but im not sure if that means I need the surgery. The surgeon told me that if I am able to get back to it then I won't need it but if I can't I need surgery. I'm a little stressed out because this will be my first year in college (lucky I stayed close to home) and I have some classes in person and some online. Surgery will change and make it a little more stressful

      if you can play football without your knee running away then it's ok but be careful if you want to perform in this sport because I told you I don't know what is the percentage of total acl rupture, talk to your doctor and determine what is more good for you, don't forget and the psyche plays a very important role.

    The thought of not being able to hike for a while keeps me awake at night, its 6am and i can't sleep. I had a partial tear 6 months ago and i feel slight pain while walking, exercise helps and the pain disappears shortly after. I think the cold of the winter is affecting it way more, 3 months after the injury i used to play football and i had no problems, most likely because it was summer. But i see people having to deal with worse injuries than mine so at least i can't complain that much. Be careful guys :(

      @@competitiveprogramming316 After 10 months it's 90% back to normal naturally, i can't push myself past 20 kms a day because the pain starts kickin' in but it gets better and better by every month.
      The first two months were really bad, crutches and shit, now i'm like a new guy.

    Here's a subtlety differently version of this question. Do you need a second ACL graft surgery if the first ACL graft surgery is partially torn?

    I had a partial ACL tear 2 years ago. I never had any swelling but sometimes i used to have some discomfort lying on the floor and trying to straight my leg. My doctor advised me some exercises I kept doing those and that discomfort vanished away. Now I am in the gym doing squat but only in a limited range of motion. I suggest you all to stay away from leg extension machine because it puts a lot of force on the ACL.

    I had a partial ACL tear and bucket handle lateral meniscus tear playing volleyball in 2015 after I hyperextended my left knee running, and subluxed my knee cap, they had to reset my knee cap when I had reconstructive surgery 4 months later. I only experienced joint instability one time after the initial tearing. I was walking down a hill and at the time I didn’t know that’s what happened, it just felt like I tripped. I played multiple sports so they suggested surgery. I still haven’t gained back my full extension. It stays at about 3 degrees. In April 2018 I had a full tear of my other ACL, MCL, and medial meniscus playing racquetball. I had surgery within a month and I’m 5 months post op and have full range of motion in that knee. I have almost no problems with the recovery of my second one, but I still struggle with my knee from my first surgery today

    9 1/2 weeks out from ACL complete tear and meniscus buckethandle. Surgery is easy recovering isnt. Ive got full range of motion but strength is slow to get back. Still working on it.

    I just had surgery in my Meniscus and Microfracture on 1/22. I knew that also had a partial tear in my ACL that may or may not be repaired during surgery. After surgery, I was advised that partial tear is indeed partial and does not repair.

    In 2015, I had surgery for my torn meniscus.
    It was a "complex tear", whatever that means. They never mentioned that I had a "partial tear in ACL" until I saw it on the post op report. I still have pain sometimes. My knee feels weak, and sometimes I think Id have been better off completely tearing it, so they couldve fixed it.
    Then again, if it WAS torn, and the surgeon didnt fix it(because the post op report didnt mention that it WAS fixed), then thats on the surgeon.

    I am 41, apparently partially tore my ACL and broke the tibia where the ACL enters the bone. I was skiing and took a tumble, I did not hear a pop, I heard a series of clicks, after bending my knee I heard a further series of click, like something had been pulled out and the went back in. I was advised surgery was not necessary after an MRI. I have been back to the gym for a week now, cycling machine and cross trainer. My gait is far from normal and the knee is still swollen. Starting physio this Friday, fingers crossed I can avoid the op!

    I had double meniscus surgery on 10/20/23-returned back to work 12/15/23 after PT. On 12/30/23, I re-injured my knee by just pulling a blanket off my couch (laundry day)-saw my doctor today after having X-rays and nothing abnormal was seen. Upon examining my knee, he stated my ACL could be torn and looked at my previous MRI before my surgery. It showed a partial ACL tear. He did a cortisone shot and will report back to him next week. If this does not give me relief, he will do another MRI and then advise what would be the next steps. Hope this works so I can get back to strengthen my leg/knee and be pain free- this pain is worse for me than the 2 torn meniscus.

    I have a partial tear but the doctor suggest to have an operation if i really want to go back to basketball. if not operation is not needed anymore. he suggest me to have a therapy or do sports without sudden change of direction like biking or swimming

    Give me a suggestion , Do i need Surgery? And in how many days it would reciver?
    My knee report is:
    1.Avulsion fracture of anterior tibial spine at ACL attachment with minimal superior
    displacement of fracture fragment.
    2. Fracture of central and posterior part of medial as well as lateral tibial plateau.
    3. Trabecular fracture and marrow edema involving tibial metadiaphysis and head-neck of fibula.
    4. Partial tear of ACL.
    5. Moderate lipohemartrosis.
    6. NO meniscal tear.

    I’m 5 weeks out from when I injured my knee. X-ray showed a segond fracture of the tibia and mri showed a high grade partial acl tear. My orthopedic doctor explained that his experience with the partial tears is that they vary case by case; even though the mri showed some of the fibers are still in tact, they are too stretched out to do their job properly.
    I’ve been in physical therapy for 4 weeks now and have full range of motion and the swelling is down to basically normal now. Starting to do more agility work during pt with no issues or feeling of instability yet. The doctor, as of now, feels confident that I will avoid surgery due to no feelings of instability (yet) and his physical exam of my knee. He said his physical test (Lachman test) was symmetrical in my injured knee when compared to the healthy knee.

      Yes I am doing good. Don’t have any issues with the knee that prevents me from living my life the way I did before. I definitely wouldn’t say my knee is 100%. It gets sore pretty easily, but no pain with it thankfully. Who knows how it will be when I get older, but for now i feel pretty good with how it feels.

    I’m fourteen and I partially tore my ACL playing rugby about two months ago, and instead of surgery I did physical therapy for a month. I have a follow up doc appointment in three days to see if I’ll need surgery or if it’s strong enough to go back into rugby. I’m pretty nervous since I’ve never gotten a surgery before.. 😥

    MRI said I have a SEVERE partial tear... doctor said I need no surgery because I am not an athlete. My job is on my feet walking pushing pulling and stopping with a HUGE HEAVY art at Kroger as a PickUp lead. My knee gives out just walking

      I had a buckling knee too. I kept telling my doctors. I went to 36 therapy appointments over course of 2 years. they did not tell me to wear a knee brace to stop the knee from giving out. Now, I have so much cartilage damage that I cant use stairs. I reduced my work hours by half putting me into poverty. Be careful. Take care of knee instablilty

    No still it gets better then it goes back to the same and therapy we can do anything with The strength because it gets swollen and it starts hurting

    Helpful stuff! My story's that I went for a small trekking trip a year ago and I had put on really wrong shoes (very soft bouncy sole). During decent, I could feel a lot of stress on my knees. The next day I experienced acute pain while climbing down the stairs. The pain diminished but didn't completely go away in the next couple of days. I ignored the incomplete recovery and started running. I probably had chondromalacia patella. Since I ignored incomplete recovery, I had more injuries - partial ACL tear and mild meniscus tear. The recent MRI says ACL tear is partial and meniscus tear is gone now. So, for almost 9 months I have been suffering with these injuries and have been very slowly recovering from these. What has helped, no matter how insignificant, is basic thigh muscle exercises and collagen type ll tablets. I hope to be fine in the next couple of weeks.

      I fell from my scooty n got low grade partial tear.. should I get operated..it's been a month n I can't walk normally.. difficulty in straightening leg.. n since I have other damages like pattella burst and joint effusion..doc asked to wait for another month to decide extent of acl injury..or whether it needs operation.. can you suggest something

    I don't know why my doctor told for ACL reconstruction my knee completely stable and unfortunately meniscus grade 3 tear I told the doctor I need meniscus repair he said of repair they will again tear I'm very confused now there not good surgents around

    I am currently 29 years old and I had complete ACL tear in 2009. I was consulting ayurvedic doctor and had several medications from them. I lived with torn ACL until 2017 doing all the activities. After having severe pain and felt unstable many times. Then I had a ACL reconstruction surgery in 2017. Today, in 2023 I had a motorbike accident with several injuries. I felt the same instability in my knee. Gone through MRI scan it shows near complete torn of ACL Graft again!! I am now confused what to do.

    This my mri report . Is it possible to recover without surgery .configuration. IMPRESSION: MRI left knee joint reveals -• Diffusely hyperintense ACL with near complete tear in its upper and mid part with mild laxity of PCL noted. PCL however show normal continuity, thickness and signals.• Mild to moderate joint effusion also extending to retro and suprapatellar recesses. Significant fat and soft tissue edema with inflammatory changes seen mainly in popliteal fossa region.• Marrow contusion with edema seen in upper end of tibia.

    So I got an MRI on my knee and it said I have a slight Acl strain.. the orthopedic Dr examine my knee and I did not have any pain. It only hurts when I jump. I can’t explode off the ground. I play football and i I get sharp pain on my knee when I pass block ;(

    i have partial acl tear and medial meniscus tear 1 month ago i can walk and doing simple exersice to buid muscle arround knee and i have swollen after long walking sometime pain start when knee is bend for long time and i wish my injury get better after two next working out month.

    Meh, fracture of the postero lateral tibia plateau (but everything still in place), lowgrade ACL (partial tear), MCL injury PCL intact. Waiting for treatment plan this thursday. Meh :(:( 36 yo skier / rock climber. Oh and the posterior lateral meniscus has a complex tear.. something with radial and oblique.. but don't know about that ;)

    My MRI says 75 % tear and I'm afraid to get under the knife. My doctor asked me to go for surgery only if there are symptoms of buckling out and pain, which are not there. I want to start running as I was on a weight loss journey and had already lost nearly 50 lbs, but I'm scared and have confidence issues with my injured knee. It's such a dilemma. My doctor said, even if I go for a surgery, it would be good as I'm only 22 and a full recovery would be possible. But then I've also seen these things fail and I don't like the prospects of that.

    Dear, i have knee injury by playing seasonal football almost 25 days.
    My MRI report shows that , Partial Thickness Tear Of ACL.
    Now i have decreased the pain but my right knee cant move or ban fully because of pain .
    In this situation Dr. Advised to operation of torn ACL.
    Please give me your suggestion
    I should go for surgery or not ?

      Try islamic prayer brother...we tend to pray sitting on a chair when we get a knee injury...but that is wrong for me when I partially tore my acl..prayer movements helped me a lot and now I can fully bend my knee...at first it was painfull and hard...but after sometime the pain disappeared

    I was clinically diagnosed by knee doc that I tore my ACL and surgery was going to be my only option most likely. I was able to run on it, play some hockey on it (both obviously not 100% but still decent) and it never once gave out on me ever even day to day. He said getting an MRI is something I should do if I chose surgery so they can see what else is damaged in the knee--I chose not to. I ended up going to a PRP doc who did some tests and thinks it's only a partial tear. I ended up doing the PRP injections 2 weeks ago so far, so good.

      Hey bro same here I had my tear 10 months ago. And all I did was physical therapy. I’ve been playing soccer for a month now and 2 weeks ago my knee got swollen but then only lasted 2 or three days and went back Into action the following week.

    Sir my MRI report says
    Anterior cruciate ligament moderate to high grade tear noted.
    Linear tear body and posterior horn of the medial meniscus.
    Subtle subtle subchondral cyst formation with edema lateral to the intercondylar notch of the tibia.
    Mild lateral subluxation of the patella.
    What is the solution for this please give me your suggestions

    my son is diagnosed with high grade partial acl tear and was advised for acl repair instead of acl reconstruction. the decision i think was made as such since my son after days from the injury can already walk straight,the swelling already subsides and pain is almost gone

      Hi my son underwnt acl reconstruction, because when they try to repair his acl , the ligament got easily torn and is impossible to repair. So the doctor decided to proceed with reconstruction. Presently he is more than 1 year from his surgery and is already back playing basketball. Rehab is very crucial in his recovery

    My ACl partial teared my doctor suggested not to play sport any more and not claiming steps to much and you dont need sergury but my knee it is not stable during sport do i have to do sergury?

    Dr. Geier,
    First, thank you taking time to educate the public on knee injuries.
    Secondly, I injured my knee in Dec.2016. I have done physical therapy, aquatic therapy, two cortisol injections, and two MRIs. Because of the continued swelling, instability, and pain my doctor decided to perform scope. Currently, I'm 8wks post-op. My doctor found a partially torn ACL which wasn't seen on either MRI. My doctor did not repair the tear nor did he debris the knee. Basically, nothing was done to the knee. Once again, I'm in physical therapy. However, my therapist stated because the tendon is catching the pocket behide my knee and the amount of scar tissue, therapy might not work. My therapist massages knee but it isn't moving or progressing. Can you suggest (of courses not diagnose) any other options? Thank you.

      Brother my MRI reprot also does not show any partial tear.....and i never had any swelling as well....but i a got a knock on my knee 1 and a half year ago.....and till today i feel something is wrong in my right knee....though it may be a very very partial tear....but still in reports...it is not shown..what should i do??     Living with and ACL partial tear grade 2 can lead to degenerative cartilage or other consecuences? Im having surgery in 7 months but im afraid of degenerate or tear other ligaments in my knee, im 22 years old, i dont wanna have arthritis in my 20s :(

    I got ACL tear 8 weeks ago. Ever since I have used crutches, and wore a knee brace for 6 weeks. It has been now 8 weeks and I still can't walk. I go to physiotherapy to train my knee. My doctor said they don't know if I need surgery, but I hope not. I am really scared, but what will happen if I need a surgery and I don't want to? please answer!

      @@helloworld269 if your young and not in terrible pain. And you can function decently well, I might say let it ride. I thought after surgery itd be better buuuut nope not really.
      Hell it's been a while since this post and I still don't have full range of motion. I cant squat or sit on my haunches anymore. I still get sharp awful pain where it hurt before the surgery.

      @@Maliq993 My recovery was a lot faster. My therapy was intense. In addition, I worked myself outside of physical therapy. Mental health conditions you. Now, I'm glad I skipped surgery.

      I got a mayor tear .torn meniscus and torn my cuadriceps. My first dr was a disaster. Said me is just muscule problem. Then I got blood in my knee and staf infection two surgeries for clean the pus on my knee and infection on my blood. Wait 6 months for surgery. Then the new dr clean scart tissue and fix my meniscus and do manipulatiob. 6 weeks of PT I got a ROM of 115 but my muscules are still weak. And my dr. Said me other 6 weeks of PT and dont want to make the surgery of ACL. He said that I have to learn to walk with my ACL that is 10 %. Is it possible?

    I tore both sides of the meniscus and a partial ACL tear they remove the meniscus tears but didn't repair my ACL tear still having a lot of problems my knee has been catching and swelling and can't seem to straighten it all the way and a lot of pain in the back of my knee the doctor don't want to do anything about it I don't know what to do I'm a pretty active person but I can't do most of the things I want to do any suggestions

    Hi Doc. I do have a question if you can aswer it to me , i do have partial tear Acl and Pcl with tear of medial Meniscus, my doctor said he doesn"t advice me for surgery specially i am above 40, and there is alot of complications after surgery specially arthritis, and just mild weight training and exercise and that will help the knee, is it true what he had said, no need for surgery ?

    Hello Dr. David ! i am a patient of ACl knee replacement , i'm from Cambodia , i had acl surgery around 8 weeks ready . i could walk step by step but i want to ask you about my knee , After i had radiography for knee , it had a small effusion ( have water inside the knee but i didnt hurt it just normal ) could i know when the effusion of the water inside my knee disappear ? thanks so much Dr. David.

    Is a Chronic Grade 2 tear the near full tear you mentioned, or just a small tear?

      I had that. I now have cartilage damage and daily pain. I walk with a limp. It is hard to work. Do not neglect it. Wear a brace, avoid heavy weight.

    Hello, I have a partial ACL tear. The joint is stable. It has been 4 months since the injury. Sometimes I feel that something is not right in the knee. I engage in BMX sports, which involves tricks on a small bicycle. I've started working out in the gym to strengthen my leg muscles. However, I'm very afraid to return to sports because it involves rotations and falls. Is it worth having surgery in this case?

    Ive been diagnose with acl partial tears but the doctor said i need to go under acl surgery im from philippines but he examine my knee he said that the acl problem thus not compramised totally my knee injury can you help me doc, for second opinion doc, if do i need to go for surgery thanks.

    I have a small tear “ scar tissue” on my acl” I’ve been doing therapy for almost 4 months and my knee feels the same. Is it time for surgery ?

      As a physical therapist I can say you, that ligament doesn't have ability to recover fully. The partial tear of ligament can't grow back to natural state. Than you've got instability of body, not only of knee. Your brain has changed sense of muscle activity of damaged leg and you can overuse healthy leg, what can lead to problems with spine or hips. Physical therapy is good to strengthen inactive muscles of damaged leg and prevent other problems. Your ligament won't be same without the surgery, but with p. therapy, you can learn how to accept the state with tear. And I think, that if it isn't very restrictive for you, you should continue with PTherapy.

    Sir my acl is partially tear 2 weeks back .. my doctor suggest me surgery. My age is 27 . Can I wait because I am not ready for surgery

      Hii ankush I got the same low grade partial acl tear.. n one doc suggested surgery.. m really scared.. whereas other one asked to wait for a month to clear up other issues like joint effusion and patela burst.. to see if I really need surgery

    Hi Can u suggest me. Doctor suggest me surgery u wll do it.. ..report is no 1. horizontal tear in posterior horn of medial meniscus. And no 2.ACL partial tear...

    My MRI said low-grade linear partial longitudinal tearing within its proximal to mid fibers. My Dr recommended no surgery. I also suffered a mildly depressed intraarticular fracture within the posterior lateral tibial plateau in this same injury, which he also recommended no surgery. I’m worried about this recommendation of no surgery as everything I’ve read shows surgery is necessary for both parts of the injury.

    My mri says i completely tore my acl but 4 doctors examine my knee like lachman test and other tests and all 4 said that my knee is stable. What should i do, should i go to surgery ???

      We have same situation.MRI said I tore ACL completely but when I went to physiotherapist,she advised me to do MRI again before going surgery because she felt like my ACL is ok.
