It will not work because of the new update. Replace randomizer gate with modify gate. Set the mode to 'clear “last activated by”'. It will work when you do these.

use an and gate, it only becomes activated when all linked stuff are activated. so, link the item triggers to the and gate and boom. you can link the and gate to the gates that make the second floor open.
Add powered blocks to block the door and link the 3 item use triggers to an and gate and the and gate to a not gate and a not gate to the powered block
