- This is the oldest known Touhou music fanwork, a MIDI cover of The Grimoire of Alice, the extra boss theme from 東方怪綺談 ～ Mystic Square, by 紫電. It was released in January 2000, beating other fan-made covers, arrange versions or remixes by over a year.
This file was made for the SC-55 Sound Canvas, and it was contributed to ZUN's site back when he was accepting music submissions. The SC-88 and SC-88Pro have compatibility issues with MIDI files made for the SC-55, but this is how ZUN would have heard it.
The first Touhou arrange was probably made by Y.GORO!! in late 2001.

You appear to know some of the older Touhou fan stuff, so I wonder, do you know anything about this particular Fanime based on PCB? Judging from the animation style and quality it looks like one of the oldest high quality fanimes out there.

I think this is my first time seeing this one, sorry. Marisa is wearing her IN clothes here, so this was released in 2004 or later. The video quality looks like a typical video from the early days of Niconico though (2007+), so maybe it's not that old.
The first fan-made animation likely predates Niconico, and it was probably uploaded to a temporary file host and posted on a message board or personal site, so it might be difficult to track down.

So is this the first Touhou Doujin work?
Probably not. it's a lot easier to make a 4koma than a music arrangement. Some manga or other form of doujin work likely existed before this.

i believe the video in question was just an arrangement of tabula rasa, the op claimed it to be the first 2hu arrangement.
The arrangement in question is archived and stored in a recreation of the old site zun uses.
I lost the link, the uploader was marisa nya.
