What did he do wrong in the last point because that happens to me all the time?

He wasn't steady with his feet, and the racket was open.
He started running a bit late for the lob, but could've got there and set earlier if he tracked backwards quicker. Then before the bandeja when he brings his racket behind his head, he opens it right up completely flat and pointing to the sky, which then forces him to pat/push the bandeja top to down too sharply
