It’s not super hard to tell them apart. I just pay attention their actions.
In the episode during the first time they’re seen as babies. One of them was sucking on some sort of screw. That would clearly be Donnie. And him sucking stuff could maybe explain the gap in his teeth.
Couple minutes after that another one crawled to Splinter and touched his katana. No doubt that was Leo.
Then after that Splinter was taking caution in case the Kraang drone nearby found them. It didn’t detect anything and all seemed fine. Until one of the turtles let out a huge burp after eating. Who else but Mikey.
And obviously Raph is the one who got run over by the cart which made his plastron crack. But how I noticed it was Raph is that he was trying to be mean ( but was too cute for that) and was being a little aggressive with his brothers.
The turtles also have different head shapes (is more obvious when they're older) and different shades of green. Mikey is the lightest, Donnie is the second lightest, Leo's kinda in the middle and Raph's green is the darkest.

How was splinter able to tell them apart when they were so little I wonder
Like parents which can tell their twins apart. They have different personalities and expressions, even though they look virtually the same.
Sometimes it's just very tiny things about appearance too. Like one have hair that's a bit longer or smoother, or have different eye colour shades, height, one being a little thinner than the other, or how their voice is.
In Splinter's case, it was most likely the way they act and colour as Ralph was dark than his siblings.
