    Great content ! I wonder if meeting the need for a 4 day window to schedule an outpatient follow up from discharge would create increased burden on a PCP’s already busy workload. Very good for the patient it seems.

    Chenmed also has community community nurse case managers as you mention - they also play a critical role to prevent hospitalizations.

    I work in capitated medicine group and we know when patients are admitted, make sure they get the hospitalist that we want, specialists that we want, and come in for followup as soon as they are discharged. We will even send a car for them and bring them to the office for the followup.
