You know I’ve seen this episode like 10 times and I never once thought of that. I knew he appeared in the opening and I had just figured ‘Oh trying to trick us into thinking he’s real’ but I ignored the fact that quite a few of the opening shots are awful situations. poopybuthole is real because at the first scene when poopybuthole flashback to a memory it was about him and the smith family being stuck on an elevator. poopybuthole never helped them to escape the elevator situation that means he was suffering inside the elevator as rest of them. it was cousin nicky who helped them escaped that means cousin nicky was the guy everyone remembered fondly. so cousin nicky created that false memory because he's a parasite.
