    Thnx , but how penetrating trauma may lead to tension pneumothorax ?

      I don't think a penetrating trauma is typically associated with a tension pneumothorax, but to answer your question, anything that allows air into the pleural space (the area between the parietal and visual pleura, but won't allow air to exit the pleural space (this is the "one-way valve/flap" commonly referred to) can cause a tension pneumothorax. For example, here is a link to an article discussing how a misplaced dressing could lead to a tension pneumothorax. Think about this setting. Let's imagine a patient who is the victim of a stabbing. The stab wound pierces not only skin, but the lung itself. A well intentioned but misguided healthcare provider places an occlusive dressing over the entire wound taped down on all 4 sides. If air is escaping the lung and going into the pleural space when the patient takes a breath in, but a flap of lung tissue occludes that same hole when the patient goes to exale, the air has nowhere to go due to the tight occlusive dressing taped down on all 4 sides.
