I would suggest that you should also put the other side rail on her left side just to make sure that in case the pillow would slipped or detached from her back. Safety is the main concern. Thanks for sharing your videos. Very educational. Thumbs up.
Hi, Rod! Thank you for your comment, suggestion compliment! There are a several definitive reasons as to why the nurse aide/nurse aide candidate should not raise the side rail on the opposite side. One, you have to understand that these training demonstration videos for nurse aide students/candidates who are in training and/or preparing to take their state nurse aide certification exam. Candidates have to follow and perform individual steps for each testable skill. Because their skills exam is timed, you do not want the candidate to perform any additional, unnecessary steps. Doing so would take away from the prescribed time they are given to complete all 5 assigned skills. Hence, causing them to fail their exam for not completing the exam in the alloted time. Second, even if my videos were demonstrating how to perform nursing tasks in a "real life" situation, you are not allowed to raise side rails unless the person has a doctor's order for use of side rails as enablers or padded side rails for seizure precautions. CMS (Centers for Medicaid and Medicare Services) defines side rails as a restraint. OBRA (The Omnibus Budget Reconciliation Act of 1987) prohibits the use of restraints, as it is their (residents, patients, clients) to be free from restraints. The raising of side rails without a doctor's order is considered a form of abuse. Three, side rails do not keep the person safe. Many injuries and deaths have been linked to raised side rails due to patients who may be confused or spastic getting body parts trapped in the deadspace of the side rails. Thank you for the training videos. I don't know if you're aware but the updated 2019 range of motion videos for shoulder and leg and this video (positions on side) cuts off before u complete the training video.

Have a question real quick please, so the legs were uncovered to show us how to support the leg to place the pillow for support, taking the sheet off, but how will we do that during testing to not over expose our resident? Please and thank you.

Are the procedures different for different states? During training we were told to always roll the resident towards you and never away. This was done in class and it was also performed this way by the staff in the nursing home where we did our clinicals. Also, we used 5 pillows. One under the head, one behind the back, one between the knees and ankles, one under the ankles, and the last was used to support the top arm and shoulder. I've also noticed that in other CNA skill videos (yours and others) that procedures were done in a way that we were told not to do. I have my exam next Tuesday and I am beyond nervous (I have testing anxiety) especially after trying to find skill videos that matched the way we were trained. Also, any advise you can give on how to get through the test while having such an anxiety issue (I'm worried about a panic attack and freezing up) would be great.
@@Orion42Lynx Hi, Orion! If I am not mistaken, the Authorized Test Administrator for the state of Michigan is Prometric. Prometric has its own Skills List which is different from Pearson View. There may be some required skills that are similar, but may have different steps. I strongly advise you to print and/or review the Nurse Aide Candidate Handbook or skills list review from Prometric if your instructor has not given you a copy. As far as your anxiety, don't let it get the best of you! Talking through your steps while performing the skill will help ease your anxiety and nervousness. Plus, communication is one of the five indirect care skills. I'm not quite sure of the time frame Prometric has set forth for the skills exam, but usually will be between 25 to 40 minutes depending on which skills you have on your skill card. Hope this was helpful!
Should the privacy be maintained after the skill? In some of the skills, sometimes you maintain the privacy and sometimes not. When should I know when to open the curtain or maintain the privacy? Thank you for your videos
Also.... Where is the privacy blanket and the fanning the sheet down so that you don't have to unmake the bed? I know this is a teaching video, but to do all steps is very helpful for those going to take the test.

Would I be able to higher the bed while doing the skill so that I'm not having to bend down so much? Or does the bed have to stay low during the skill?
Hi, Lulu! You may raise the height of bed to a level that is comfortable for you to perform the skill (body mechanics). Just remember to lower it to a safe level before stating, "skill complete".
One question when finish position client on one side, do I put the bed rail down before ending the skill while client is facing on one side, or keep the rail up?
Hi, Noella! Goid question! The lowering of side rails is not scorable. Therefore, you may opt to keep it raised or you may opt to lower it. For time management, I instruct my students not to lower it, as some side rails can be time consuming to lower.
Hi, Marton1300! During testing, you'll be required to wash your hands with actual water and soap only once during the skill "Hand Hygiene". Any time after that, you'll need only to verbalize "washing hands" whenever a skill requires you to do so.

Hi, I’m testing out in a few days and as I’m looking through my Pearson vue handbook for this skill and it says “before turning lowers head of bed”? Can you explain exactly what that means?
Hi Ms. Julie, I had a quick question about this skill. when putting the pillow underneath the leg, would pull down the blanket when putting the pillow underneath the leg, or would you just keep the patient covered and slide the pillow underneath the leg. Hi, Maddy! It is okay for you to uncover the leg. Just make sure you recover the leg after placing the pillow. Also, do not pull the sheet downward. Because you're working with the legs only while placing this pillow, you will want to uncover the legs by either moving the top sheet to the side or pulling the sheet upward from the foot of the bed. In the booklet it says place pillows under the arm first then back then legs. So for testing do we go in that order or how u just did it in the video? (how u did it makes more sense then what the book says)

If we forget to adjust the head pillow as our first pillow, but still adjust it before the skill is complete will that count? or does it have to be the first pillow adjusted?
Hi, Luis! I train my students to give the person the call light during opening procedures just in case they forget to do so before stating "skill complete ". During testing, you may be somewhat nervous and forget to do some things. This is something you do not want to forget to do!
Hi, Merry! You're letting your nerves get the best of you. Because you've taken the test before, you should be familiar with the process, which is a good thing. You probably have test anxiety. Talk your way through every step of every skill you are required to perform. Take deep breaths and focus only on what you are doing. You'll get it next go around.

Great video. I know you verbalized washing your hands after the skill but shouldn't you have stated that before started the skill as well to identify that this is the proper way of completing the skill by washing hands before and after a skill. Maybe you can state that in the video in the beginning before starting next time so some won't get confused. Thank you for the visualization!
Hi, Queens! Thanks for your comment. My videos demonstrate "testable" skills, not how or what you need or should do in real life when you are performing nursing skills. Too many times, folks become confused when they integrate what is done in real life with what is required of the candidate to do during testing. If you review this skill in your NA Candidate Handbook, you will see that the only time the candidate is required to verbalize "washing hands" is at the end of the skill before stating "skill complete".

I am a RN student and in the process of obtaining my Bachelor's degree and have seen many videos that explain along with reading according to nursing boards stating that is good to verbalize before and after just like it was the real thing but I understand that's how you train. @@NURSEJAR but in your video where you used the gait belt to assist a patient, you pulled the curtains and then washed your hands before gathering the belt and touching the patient. That seemed more accurate and what I've been studying in my CNA practice exam book for all states.

@@tawakkul3 Hi! In all my training videos, unless they are super clips, I perform "opening procedures" which include safety checks and providing privacy. In the videos you are referencing, "Positions on Side" and Assists With Ambulation Using Transfer Belt", I did just that...performed opening procedures. So, I'm not quite sure what you are looking at to say I did not provide privacy before performing this skill, Positions on Side. I've reviewed both videos, and my opening PROCEDURES are the same for both in regards to safety, privacy, and infection control.
Hi, Samson! Thank you for your comment. Please, keep in mind that these videos are solely for nurse aide candidate training. Candidates have to perform steps verbatim as listed in their candidate handbook. During testing, the only time the candidate has to wash their hands with running water and soap is during the skill "Hand Hygiene". Anytime after that, if the skill requires, the candidate needs only to verbalize "washing hands". As far as "identifying" the resident, again, for testing, this is not a required step. Candidates should not perform unneccessary steps, as this will add time into their skills exam. The candidate wants and needs to be able to shave time whenever possible.

Do you have to wash your hands before starting this skill?
Are the procedures different for different states? During training we were told to always roll the resident towards you and never away. This was done in class and it was also performed this way by the staff in the nursing home where we did our clinicals. Also, we used 5 pillows. One under the head, one behind the back, one between the knees and ankles, one under the ankles, and the last was used to support the top arm and shoulder. I've also noticed that in other CNA skill videos (yours and others) that procedures were done in a way that we were told not to do. I have my exam next Tuesday and I am beyond nervous (I have testing anxiety) especially after trying to find skill videos that matched the way we were trained. Also, any advise you can give on how to get through the test while having such an anxiety issue (I'm worried about a panic attack and freezing up) would be great.

@@Orion42Lynx Hi, Orion! If I am not mistaken, the Authorized Test Administrator for the state of Michigan is Prometric. Prometric has its own Skills List which is different from Pearson View. There may be some required skills that are similar, but may have different steps.
I strongly advise you to print and/or review the Nurse Aide Candidate Handbook or skills list review from Prometric if your instructor has not given you a copy.
As far as your anxiety, don't let it get the best of you! Talking through your steps while performing the skill will help ease your anxiety and nervousness. Plus, communication is one of the five indirect care skills.
I'm not quite sure of the time frame Prometric has set forth for the skills exam, but usually will be between 25 to 40 minutes depending on which skills you have on your skill card.

How will we be able to see if we have the pillow in the right position if we can’t uncover the persons legs while positioning the pillow??
Hi, Slaylike! You are allowed to uncover the person's legs while placing the pillow. Just remember to recover the person once you are done.
Would I be able to higher the bed while doing the skill so that I'm not having to bend down so much? Or does the bed have to stay low during the skill?
Hi, Lulu! You may raise the height of bed to a level that is comfortable for you to perform the skill (body mechanics). Just remember to lower it to a safe level before stating, "skill complete".
One question when finish position client on one side, do I put the bed rail down before ending the skill while client is facing on one side, or keep the rail up?
Hi, Noella! Goid question! The lowering of side rails is not scorable. Therefore, you may opt to keep it raised or you may opt to lower it.
For time management, I instruct my students not to lower it, as some side rails can be time consuming to lower.

Hi, Marton1300! During testing, you'll be required to wash your hands with actual water and soap only once during the skill "Hand Hygiene". Any time after that, you'll need only to verbalize "washing hands" whenever a skill requires you to do so.

Hi, I’m testing out in a few days and as I’m looking through my Pearson vue handbook for this skill and it says “before turning lowers head of bed”? Can you explain exactly what that means?
Hi, Sadee! Yes! If the HOB is elevated, you must lower it completely to position the person in supine position before turning them on his or her side.
Hi Ms. Julie, I had a quick question about this skill. when putting the pillow underneath the leg, would pull down the blanket when putting the pillow underneath the leg, or would you just keep the patient covered and slide the pillow underneath the leg. Hi, Maddy! It is okay for you to uncover the leg. Just make sure you recover the leg after placing the pillow.
Also, do not pull the sheet downward. Because you're working with the legs only while placing this pillow, you will want to uncover the legs by either moving the top sheet to the side or pulling the sheet upward from the foot of the bed.

In the booklet it says place pillows under the arm first then back then legs. So for testing do we go in that order or how u just did it in the video? (how u did it makes more sense then what the book says)

I was wondering if I should gather all my supplies at the beginning of my time or just grab everything as I go through every skill? Is gathering all your supplies at the beginning even an option? Or should I just get supplies individually as I go through each of the bedside skills?

During testing, you may be somewhat nervous and forget to do some things. This is something you do not want to forget to do!

You probably have test anxiety. Talk your way through every step of every skill you are required to perform. Take deep breaths and focus only on what you are doing. You'll get it next go around.

Great video. I know you verbalized washing your hands after the skill but shouldn't you have stated that before started the skill as well to identify that this is the proper way of completing the skill by washing hands before and after a skill. Maybe you can state that in the video in the beginning before starting next time so some won't get confused. Thank you for the visualization!
Hi, Queens! Thanks for your comment. My videos demonstrate "testable" skills, not how or what you need or should do in real life when you are performing nursing skills.
Too many times, folks become confused when they integrate what is done in real life with what is required of the candidate to do during testing.
If you review this skill in your NA Candidate Handbook, you will see that the only time the candidate is required to verbalize "washing hands" is at the end of the skill before stating "skill complete".

I am a RN student and in the process of obtaining my Bachelor's degree and have seen many videos that explain along with reading according to nursing boards stating that is good to verbalize before and after just like it was the real thing but I understand that's how you train. @@NURSEJAR but in your video where you used the gait belt to assist a patient, you pulled the curtains and then washed your hands before gathering the belt and touching the patient. That seemed more accurate and what I've been studying in my CNA practice exam book for all states.

@@tawakkul3 Hi! In all my training videos, unless they are super clips, I perform "opening procedures" which include safety checks and providing privacy.
In the videos you are referencing, "Positions on Side" and Assists With Ambulation Using Transfer Belt", I did just that...performed opening procedures.
So, I'm not quite sure what you are looking at to say I did not provide privacy before performing this skill, Positions on Side.
I've reviewed both videos, and my opening PROCEDURES are the same for both in regards to safety, privacy, and infection control.

Hi, Samson! Thank you for your comment. Please, keep in mind that these videos are solely for nurse aide candidate training.
Candidates have to perform steps verbatim as listed in their candidate handbook. During testing, the only time the candidate has to wash their hands with running water and soap is during the skill "Hand Hygiene". Anytime after that, if the skill requires, the candidate needs only to verbalize "washing hands".
As far as "identifying" the resident, again, for testing, this is not a required step.
Candidates should not perform unneccessary steps, as this will add time into their skills exam. The candidate wants and needs to be able to shave time whenever possible.
