They might well have looked at the way missing children are ignored and thought that nobody would make a fuss for long. Consensus is not proof, and there is little evidence for any particular route out of her life - we just know she has gone. Given that KM was trailing cadaverine all over the place, that looks really bad for her but she has the kind of job where she encounters dead bodies and while dogs are reliable at identifying cadaverine, they cannot say whose it is. The blood found just could be Madeleine's but from another incident than her death, identification by dna profiling was dubious, I understand, as they had no certain samples from MBM to use for comparison. (!) Given these slim areas for doubt, one could try to imagine how this would all have gone down had they arranged for MBM to be sold, picked up early on in the holiday. That could explain why they don't seem to suffer the anguish of wondering what she is going through, it would explain why there was no toothbrush for her in the appartment, and would fit in with the sadness and apprehensive looks one can see in so many of the published pix and vids of MBM - as if once the twins arrive has she been supplanted and become surplus and expendable?
KM describes many times how awkward it was for her having twins plus one, how MBM was loud, cholicky, a screamer and never gave K a minute's peace. They might well have looked at the way missing children are ignored and thought that nobody would make a fuss for long. Consensus is not proof, and there is little evidence for any particular route out of her life - we just know she has gone.
There is a syndrome known as CHARGE, it can be mild or more serious but amongst the symptoms are coloboma, sticking out, sometimes low-down ears (her ears are usually covered but can be seen on the 'make-up' photo), heart defects, other malformations some visible, some internal, and there is also mention of disturbed behaviour on some sites about charge. Is it possible that the reason GM refers to MBM as 'almost perfect' when she is born, is something of this sort was detected, or maybe it was just the coloboma that made her not quite perfect?
They could run the modern DNA test now, but they are not because they are covering up for Kate & Gerry.

Gerry was seen by the Smith family carrying Madeleine to the sea shore, no doubt a boat was waiting for him there. Her body and bones are well off shore, at the bottom of the Atlantic, slowly being dissolved by the salty sea. Madeleine will NEVER be found.
